      目的分析乙型肝炎肝硬化患者肠道菌群变化与糖脂代谢指标的相关性及对肝性脑病(HE)的预测效能。方法选择80例乙型肝炎肝硬化患者为肝硬化组,80例单纯乙型肝炎患者为乙肝组,同期行体检80例健康人群为对照组。比较3组肠道菌群变化、Chao指数、Shannon指数、糖脂代谢水平,ROC分析各指标对HE的预测效能。结果与乙肝组和对照组比较,肝硬化组变形菌门和梭杆菌门占比以及空腹血糖(FBG)、2 h血糖(2hPBG)、糖化血红蛋白(HbAlc)和低密度脂蛋白胆固醇(LDLC)升高,Chao指数及Shannon指数降低(P<0.05);Chao指数和Shannon指数与Child-Pugh分级、FBG、2hPBG、HbAlc及LDLC呈负相关(P<0.05);与无HE患者比较,HE患者Chao指数和Shannon指数降低,FBG、2hPBG、HbAlc及LDLC升高(P<0.05);6项指标联合预测HE的效果最佳,AUC值为0.928。结论肠道菌群及糖脂代谢可能参与了肝功能恶化及HE并发症的发生发展,6项指标联合预测HE的效果最佳。

      To analyze the correlation between the changes of intestinal flora of hepatitis B cirrhosis and the indexes of glucose and lipid metabolism, and prediction efficiency of HE. Methods80 patients with hepatitis B sclerosis (cirrhosis group), 80 patients with simple hepatitis B (hepatitis B), and 80 healthy people in the same period (control group) in our hospital were selected as subjects. The changes of intestinal flora, Chao index, Shannon index, and glucose and lipid metabolism levels were compared among the three groups, and the prodiction value of hepatic encephalopathy (HE) was analyzed by ROC. ResultsCompared with the hepatitis B group and the control group, the liver cirrhosis group had higher proportions of Proteobacteria and Fusobacterium as well as fasting blood glucose (FBG), 2 h postprandial blood glucose (2hPBG), glycosylated hemoglobin (HbAlc) and low density lipoprotein cholesterol (LDLC), and lower Chao index and Shannon index(P<0.05); Chao index and Shannon index were negatively correlated with Child-Pugh grade, FBG, 2hPBG, HbAlc and LDLC(P<0.05); compared with patients without HE, the Chao index and Shannon index were lower in HE patients, and the FBG, 2hPBG, HbAlc and LDLC were higher(P<0.05); the six indicators combined had the best effect in prediction HE, with an AUC value of 0.928. ConclusionIntestinal flora and glucose and lipid metabolism may be involved in the deterioration of liver function and the occurrence and development of HE complications, and the combination of six indicators is the best in the prediction of HE.
