目的探讨双向呼吸道正压通气(BiPAP)联合盐酸氨溴索治疗新生儿肺透明膜病(NHMD)的临床疗效及对患儿动脉血气指标和氧合指数的影响。 方法选取80例NHMD新生儿,随机分为对照组和联合组,对照组采用BiPAP治疗,联合组采用BiPAP联合大剂量盐酸氨溴索治疗,比较两组治疗效果、动脉血气指标和氧合指数。 结果与对照组比较,联合组治疗后动脉血氧分压(PaO2)、PaO2/肺泡氧分压及pH升高,二氧化碳分压降低(P<0.05);呼吸相关指标呼吸频率、心率低于对照组,氧合指数高于对照组(P<0.05);机械通气相关参数吸入氧浓度、呼气末正压、平均气道压力均低于对照组(P<0.05)。联合组治疗总时间和并发症发生率低于对照组(P<0.05)。 结论BiPAP联合盐酸氨溴索治疗NHMD临床疗效显著,改善患儿动脉血气指标和氧合指数,值得临床推广。 |

To investigate the clinical effect of bidirectional positive airway pressure ventilation (BiPAP) combined with ambroxol in the treatment of neonatal hyaline membrane disease (NHMD) and its effect on arterial blood gas index and oxygenation index. Methods80 newborns with NHMD were randomly divided into control group and combined group. The control group was treated with BiPAP and the combined group was treated with BiPAP combined with high-dose ambroxol hydrochloride. The therapeutic effects, arterial blood gas index and oxygenation index of the two groups were compared. ResultsCompared with the control group, the arterial oxygen partial pressure (PaO2), PaO2/alveolar oxygen partial pressure and pH in the combined group increased and the blood carbon dioxide partial pressure decreased after treatment(P<0.05). Respiratory related indexes respiratory rate and heart rate were lower than those in the control group, and oxygenation index was higher than those in the control group(P<0.05). The parameters related to mechanical ventilation, such as inhaled oxygen concentration, positive end expiratory pressure and mean airway pressure, were lower than those in the control group(P<0.05). The total treatment time and complication rate in the combined group were lower than those in the control group(P<0.05). ConclusionBiPAP combined with ambroxol is effective in the treatment of NHMD. It can improve the indexes of arterial blood gas and oxygenation index in children, which is worthy of clinical promotion. |
