目的探究右美托咪定联合布比卡因通过cAMP/PKA-CREB-BDNF信号通路对大鼠坐骨神经阻滞效果的影响。方法选取SD大鼠36只随机均分为假手术组(Sham组)、布比卡因组(Bup组)、右美托咪定+布比卡因组(Dex组)和毛喉素+右美托咪定+布比卡因组(Fors组)。分别于阻滞前和阻滞后不同时间测定各组大鼠热刺激缩足反应最大效应百分比(MPE)和后肢伸姿推力(EPT)。于阻滞12 h检测各组脊髓环磷酸腺苷(cAMP)、蛋白激酶A(PKA)、磷酸化环磷酸腺苷反应元件结合蛋白(p-CREB)和脑源性神经营养因子(BDNF)的水平。结果Bup组和Fors组在阻滞后10~180 min、Dex组在阻滞后10~360 min MPE较Sham组升高(P<0.05);Bup组在阻滞后10~90 min、Dex组和Fors组在阻滞后10~120 min EPT较Sham组降低(P<0.05)。Dex组在阻滞后10~360 min MPE较Bup组升高(P<0.05);Dex组和Fors组在阻滞后10~120 min EPT较Bup组降低(P<0.05)。Fors组在阻滞后10~360 min MPE较Dex组降低(P<0.05)。阻滞后12 h cAMP、PKA、p-CREB、BDNF表达水平Sham组较其余3组升高,Dex组较Bup组下降,Fors组较Dex组升高(P<0.05)。结论右美托咪定联合布比卡因增强大鼠坐骨神经阻滞效果,并延长了感觉和运动阻滞时间,其机制可能与抑制cAMP/PKA-CREB-BDNF信号通路有关。 |

To explore the effect of dexmedetomidine combined with bupivacaine on sciatic nerve block in rats through cAMP/PKA-CREB-BDNF signaling pathway and related mechanisms. MethodsTotally 36 SD rats were randomly divided into Sham operation group (Sham group), bupivacaine group (Bup group), dexmedetomidine + bupivacaine group (Dex group) and maurolarynx + dexmedetomidine + bupivacaine group (Fors group). The maximal effect (MPE) and elevated posture thrust (EPT) were measured before and after block respectively. The level of cyclic adenosine monophosphate (cAMP), protein kinase A (PKA), phosphorylated cyclic adenosine monophosphate response element binding protein (p-CREB), and brain-derived neurotrophy factor (BDNF) were detected after 12 h sciatic nerve block.ResultsMPE in the Bup group and the Fors group was higher than that in the sham group at 10-180 min after block, and in the Dex group at 10-360 min after block(P<0.05); EPT in Bup group was lower than that in Sham group at 10-90 min after blockade, Dex group and Fors group at 10-120 min after blockage(P<0.05). MPE in Dex group was higher than that in Bup group at 10-360 min after blockade(P<0.05); EPT in Dex group and Fors group was lower than that in Bup group at 10-120 min after blockade(P<0.05). MPE in Fors group was lower than that in Dex group at 10-360 min after blockade(P<0.05). After 12 h block, the levels of cAMP, PKA, p-CREB and BDNF in Sham group were higher than those in the other three groups, the levels of those in Dex group were lower than those in Bup group, the levels of those in Fors group were higher than those in Dex group(P<0.05). ConclusionDexmedetomidine combined with bupivacaine enhance the effect of sciatic nerve block and prolong the sensory and motor block time in rats and the mechanism might be related to the inhibition of cAMP/PKA-CREB-BDNF signal pathway. |
