      目的研究促性腺激素释放激素激动剂(GnRHa)联合超声射频对子宫肌瘤患者的影响及作用机制。方法选取118例子宫肌瘤患者,分层抽样法分为超声射频组(n=57)和联合组(n=61)。超声射频组采用超声射频治疗,联合组在超声射频组基础上联合GnRHa进行治疗。观察两组子宫肌瘤体积、体积缩小率、治疗效果;采用酶联免疫吸附法检测内分泌功能指标孕激素(P)、雌二醇(E2)、促卵泡素(FSH)及单核细胞趋化因子-1(MCP-1)、存活蛋白(Survivin)、肿瘤特异性生长因子(TSGF)水平;实时荧光定量PCR法检测微小RNA指标水平。结果与超声射频组比较,联合组治疗后肌瘤体积较小,体积缩小率较大(P<0.05),内分泌功能指标及MCP-1、Survivin、TSGF水平较低(P<0.05),微小RNA指标、治疗总有效率较高(P<0.05)。结论GnRHa联合超声射频可有效改善子宫肌瘤体积,纠正患者内分泌紊乱,降低Survivin、TSGF水平,提高治疗效果。
英文摘要:
      To study the effect and the mechanism of gonadotropin releasing hormone agonist (GnRHa) combined with ultrasound radiofrequency on patients with uterine fibroids. Methods118 patients with uterine fibroids were selected, who were divided into ultrasound radio frequency group (n=57) and combined group (n=61) by stratified sampling method. The ultrasound radiofrequency group was treated with ultrasound radiofrequency, and the combination group was treated with GnRHa on the basis of the ultrasound radiofrequency group. Observe the two groups of uterine fibroids volume, volume reduction rate, treatment effect; use enzyme-linked immunosorbent assay to detect endocrine function indicators progesterone (P), estradiol (E2), follicle stimulating hormone (FSH) and monocyte chemotactic factor-1 (MCP-1), Survivin (Survivin), tumor-specific growth factor (TSGF) levels; use real-time fluorescence quantitative PCR method to detect microRNA indicators. ResultsCompared with the ultrasound radiofrequency group, the fibroids in the combined group were smaller after treatment, the volume reduction rate was larger (P<0.05), the endocrine function indexes and the levels of MCP-1, Survivin, and TSGF were lower (P<0.05), and the microRNA indexes. The total effective rate of treatment were higher (P<0.05). ConclusionGnRHa combined with ultrasound radiofrequency can effectively improve the volume of uterine fibroids, correct endocrine disorders in patients, reduce Survivin and TSGF levels, and improve the therapeutic effect.
