| 目的探讨载脂蛋白B/A1比值(ApoB/ApoA1)联合胰岛素抵抗指数(HOMA-IR)对高危型孕妇妊娠期糖尿病(GDM)及妊娠结局的预测价值。方法选取接受治疗的高危型GDM孕妇92例为观察组,同期正常妊娠孕妇92例为对照组。检测孕妇血脂[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)]、ApoB、ApoA1以及空腹血糖、胰岛素水平,计算HOMA-IR。对GDM孕妇的妊娠结局进行随访,记录不良妊娠结局发生情况。结果观察组TC、TG、LDL、ApoB、ApoB/ApoA1比值、HOMA-IR高于对照组,HDL、ApoA1低于对照组(P＜0.05)；ApoB/ApoA1比值联合HOMA-IR诊断GDM的AUC值为0.846,高于单一检测(P＜0.05),灵敏度为75.00%,特异度为80.43%；对照组无不良妊娠结局,观察组不良妊娠结局发生率为11.96%,ApoB/ApoA1比值联合HOMA-IR预测GDM孕妇不良结局的AUC值为0.892,高于单一检测(P＜0.05),灵敏度为100.00%,特异度为81.48%。结论ApoB/ApoA1比值联合HOMA-IR不仅可作为妊娠期糖尿病的诊断指标,也可一定程度上预测不良妊娠结局的发生。 |

| To explore the predictive value of apolipoprotein B/A1 ratio (ApoB/ApoA1) combined with homeostasis model assessment of insulin resistance (HOMA-IR) on gestational diabetes mellitus (GDM) and clinical outcomes in high-risk pregnant women. MethodsA total of 92 high-risk pregnant women with GDM who received treatment in our hospital were selected as observation group and 92 normal pregnant women during the same period were selected as control group. The levels of blood lipids [cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL)], ApoB, ApoA1, fasting blood glucose and insulin were detected among pregnant women, and HOMA-IR was calculated. The pregnancy outcomes of pregnant women with GDM were followed up, and the occurrence of adverse pregnancy outcomes were recorded. ResultsThe TC, TG, LDL, ApoB, ApoB/ApoA1 ratio and HOMA-IR of observation group were higher than those of control group while HDL and ApoA1 were lower than those of control group (P＜0.05).The AUC value of ApoB/ApoA1 ratio combined with HOMA-IR was 0.846 in diagnosing GDM, which was higher than that of the single test (P＜0.05), and the sensitivity and specificity were 75.00% and 80.43%. The incidence rate of adverse pregnancy outcomes was 11.96% among 92 pregnant women with GDM, and the AUC value of ApoB/ApoA1 ratio combined with HOMA-IR in predicting adverse outcomes in pregnant women with GDM was 0.892, which was higher than that of the single test (P＜0.05), and the sensitivity and specificity were 100.00% and 81.48%. ConclusionApoB/ApoA1 ratio combined with HOMA-IR can not only be used as a diagnostic indicator for gestational diabetes mellitus, but also predict the occurrence of adverse pregnancy outcomes to a certain extent. |
