| 目的探讨低分子肝素钙(LMWH)对老年慢性阻塞性肺疾病急性加重期(AECOPD)并肺心病患者红细胞形态、心肺功能、凝血指标的影响。方法采用前瞻性随机研究将本院收治的122例老年AECOPD并肺心病患者分为LMWH组62例和常规组60例；两组患者均给予吸氧、祛痰、平衡电解质、抗感染等基础治疗,LMWH组患者同时给予LMWH皮下注射治疗,疗程1周；对比两组治疗前后的红细胞形态、心肺功能、凝血指标的影响。结果治疗后,LMWH组患者的PaO2、SaO2、PT、APTT高于常规组(P＜0.05),LMWH组患者D-D测定值低于常规组(P＜0.05)；治疗后,LMWH组患者的Borg评分、棘形红细胞计数、RDW测定值低于常规组(P＜0.05)。结论LMWH治疗老年AECOPD并肺心病患者对于改善高凝状态、血气水平、红细胞形态、减轻呼吸困难程度具有一定的作用。 |

| To investigate the effect of low molecular weight heparin calcium (LMWH) to erythrocyte morphology, cardiopulmonary function and coagulation parameters in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and pulmonary heart disease. MethodsA prospective randomized study was used to divide 128 elderly patients with AECOPD and pulmonary heart disease in our hospital into LMWH group with 62 cases and routine group with 60 cases ; Both groups of patients were given basic treatments such as oxygen inhalation, expectoration, electrolyte balance, and anti-infection. Patients in the LMWH group were given LMWH subcutaneous injection at the same time for 1 week; The indexes of erythrocyte morphology, cardiopulmonary function and coagulation parameters before and after treatment were compared between the two groups. ResultsAfter treatment, PaO2, SaO2, PT, APTT of patients in LMWH group were higher than those in conventional group (P＜0.05), and D-D values of patients in LMWH group were lower than those in conventional group (P＜0.05); After treatment, the Borg score, spinous red blood cell count, and RDW of the LMWH group were lower than those of the conventional group (P＜0.05). ConclusionLMWH treatment of elderly patients with AECOPD and pulmonary heart disease has a certain effect on improving hypercoagulable state, blood gas level, red blood cell morphology, and reducing the difficulty of breathing. |
