      目的探讨慢性阻塞性肺疾病(COPD)患者血清组织蛋白酶B(CTSB)、组织蛋白酶S(CTSS)表达水平及临床意义。方法选取156例COPD患者(COPD组)以及60例健康人群(对照组)为研究对象。比较两组受试者血清CTSB、CTSS表达水平及不同病情严重程度、不同肺气肿严重程度时COPD患者血清CTSB、CTSS的表达水平。ROC曲线比较血清CTSB、CTSS对COPD的单独诊断和联合诊断效能。结果COPD组血清CTSB水平低于对照组(P<0.05),血清CTSS水平高于对照组(P<0.05)。COPD病情严重程度Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级患者血清CTSB水平依次降低(P<0.05),血清CTSS水平依次升高(P<0.05)。肺气肿0级、1级、2级、3级COPD患者血清CTSB水平依次降低(P<0.05),血清CTSS水平依次升高(P<0.05)。血清CTSB、CTSS两者联合诊断COPD的诊断效能高于CTSB、CTSS单一诊断效能。结论COPD患者存在血清CTSB水平降低和CTSS水平升高,且其血清CTSB、CTSS水平与病情严重程度及肺气肿严重程度相关。

      To investigate the expression and clinical significance of serum cathepsin B (CTSB) and cathepsin S (CTSS) in patients with chronic obstructive pulmonary disease (COPD). MethodsA total of 156 COPD patients (COPD group) and 60 healthy people (control group) were selected as the research objects. The expression levels of CTSB and CTSS in serum of the two groups were compared, and the expression levels of CTSB and CTSS in serum of COPD patients with different disease severity and emphysema severity were compared. ROC curve was used to compare the diagnostic efficacy of serum CTSB and CTSS alone and in combination. ResultsThe serum CTSB level in COPD group was lower than that in control group (P<0.05), serum CTSS level was higher than that in control group (P<0.05).The level of serum CTSB in COPD grade Ⅰ, Ⅱ, Ⅲ and Ⅳ patients decreased successively (P<0.05), serum CTSS levels increased successively (P<0.05). The levels of serum CTSB in patients with grade 0,1, 2 and 3 COPD were successively decreased (P<0.05), serum CTSS levels increased successively (P<0.05). The diagnostic efficacy of serum CTSB and CTSS combined in the diagnosis of COPD was higher than that of CTSB and CTSS alone. ConclusionThe levels of serum CTSB were decreased and CTSS were increased in COPD patients, and the levels of serum CTSB and CTSS were correlated with the severity of disease and emphysema.
