      目的探讨血清脑源性神经营养因子(BDNF)与颅脑损伤患者病情及预后的相关性。方法选取本院收治的75例脑损伤患者作为研究对象,本院体检正常的36例成年志愿者作为对照组。根据格拉斯哥昏迷评分(GCS)情况将患者分为重型组、中型组以及轻型组,根据格拉斯哥预后评分(GOS)情况将患者分为预后不良组及预后良好组。分析血清BDNF水平与伤情及预后的相关性。结果重型组、中型组伤后各时间点及轻型组伤后7天、14天BDNF明显低于对照组(P<0.05);与重型组比较,中型组、轻型组伤后1天、7天、14天的血清BDNF水平均升高(P<0.05);轻型组患者伤后1天、7天、14天血清BDNF水平显著高于中型组(P<0.05)。轻、中、重型患者组内不同伤后时间点的血清BDNF水平比较差异具有统计学意义(P<0.05)。预后不良组与预后良好组患者血清BDNF水平在不同伤后时间比较差异具有统计学意义(P<0.05);通过Spearman相关性分析,结果显示伤后1天、7天、14天血清BDNF水平与患者GCS评分呈正相关(r=0.751、0.673、0.727,P<0.05),与伤后3个月GOS评分呈正相关(r=0.510、0.491、0.573,P<0.05)。结论不同伤情及预后颅脑损伤者血清BDNF水平不同,可作为临床上对颅脑损伤患者伤情及预后情况的早期判断指标之一。

      To investigate the correlation between serum brain-derived neurotrophic factor (BDNF) and the condition and prognosis of patients with craniocerebral injury. Methods75 patients with brain injury in our hospital were selected as the research objects, and 36 adult volunteers with normal physical examination in our hospital were selected as the control group. According to Glasgow Coma Scale (GCS), the patients were divided into severe group, medium group and mild group. According to Glasgow Outcome Scale (GOS), the patients were divided into poor prognosis group and good prognosis group. The correlation of serum BDNF concentration with injury and prognosis was analyzed. ResultsThe DNF of severe group, medium group and light group were significantly lower than that of control group, 7 d and 14 d after injury (P<0.05); Compared with the severe group, the serum BDNF levels of the medium group and the light group increased at 1d, 7d and 14d after injury (P<0.05); The level of BDNF in the light group was significantly higher than that in the medium group at 1d, 7d and 14d after injury (P<0.05). There was significant difference in serum BDNF concentration among mild, moderate and severe patients at different time points after injury (P<0.05). There was significant difference in serum BDNF level between the poor prognosis group and the good prognosis group at different time points after injury (P<0.05); Spearman correlation analysis showed that serum BDNF levels were positively correlated with GCS scores at 1d, 7d and 14d after injury (r=0.751,0.673 and 0.727, P<0.05), and positively correlated with GOS scores 3 months after injury (r=0.510,0.491 and 0.573, P<0.05). ConclusionThere are different levels of serum BDNF in patients with different severity and prognosis of craniocerebral injury, which can be used as one of the early indicators to judge the severity and prognosis of craniocerebral injury.
