目的探讨尾侧型同源转录因子2(CDX2)、肝细胞核因子4α(HNF4α)、性别决定相关基因簇2(SOX2)蛋白联合检测对幽门螺杆菌(Hp)感染相关高危胃癌的诊断价值。方法选取胃癌患者(胃癌组)及萎缩性胃炎患者(胃炎组)为研究对象。免疫组化检测两组CDX2、HNF4α、SOX2蛋白表达情况,13C-尿素呼气试验检测Hp感染情况。比较两组CDX2、HNF4α、SOX2阳性表达率及Hp阳性率。采用受试者工作特征(ROC)曲线分析CDX2、HNF4α、SOX2及三者联合对Hp相关高危胃癌的诊断效能。结果胃癌组CDX2及SOX2阳性表达率低于胃炎组(P<0.05),HNF4α阳性表达率高于胃炎组(P<0.05)。胃癌组Hp阳性率高于胃炎组(P<0.05)。两组Hp阳性者CDX2及HNF4α阳性表达率高于Hp阴性者,而SOX2阳性表达率低于Hp阴性者(均P<0.05)。CDX2、HNF4、SOX2对Hp相关高危胃癌的诊断效能低于三者联合的诊断效能。结论胃癌组织中CDX2、HNF4α、SOX2蛋白对Hp相关高危胃癌具有较高的诊断效能,且三者联合可提高灵敏度。 |

To investigate the value of caudal type homeobox transcription factor 2 (CDX2), hepatocyte nuclear factor 4α(HNF4α) and sex determining region Y-box 2 (SOX2) in the diagnosis of high-risk gastric cancer patients associated with Helicobactor pylori (Hp) infection. MethodsPatients with gastric cancer and with atrophic gastritis were selected as the research objects. The expression of CDX2, HNF4 α and SOX2 protein in tissues were detected by immunohistochemistry. The Hp infection was detected by 13C urea breath test. CDX2, HNF4 α and SOX2 were compared between gastric cancer and atrophic gastritis. The positive expression rates of CDX2, HNF4 α and SOX2 protein in gastric cancer/atrophic gastritis patients with different Hp infection status were compared. Receiver operating characteristic (ROC) curve was used to analyze the diagnostic efficacy of CDX2, HNF4α, SOX2 and their combination in Hp related high-risk gastric cancer. ResultsThe positive rates of CDX2 and SOX2 in gastric cancer group were lower than those in gastritis group (P<0.05), the positive expression rate of HNF4α was higher than that in gastritis group (P<0.05). The positive rate of Hp in gastric cancer group was higher than that in gastritis group (P<0.05). The expression of CDX2 and HNF4α in Hp positive tissues of the two groups was higher than that of Hp negative, the positive expression rate of SOX2 was lower than that of Hp negative (P<0.05). The diagnostic efficiency of CDX2, HNF4 and SOX2 in Hp related high-risk gastric cancer was lower than that of the combination of the three. ConclusionThe abnormal expression of CDX2, HNF4α and SOX2 protein had high diagnostic efficiency for Hp related high-risk gastric cancer, and the combination of the three could further improve the sensitivity. |
