      目的探讨全身麻醉复合颈浅丛阻滞麻醉对甲状腺术后躁动和术中甲状腺素水平的影响。 方法随机将行甲状腺手术患者84例均分为两组。对照组采用全身麻醉,观察组采用全身麻醉复合颈浅丛阻滞麻醉。比较两组麻醉诱导前(T0)、气管插管后1 min(T1)、切开皮肤时(T2)、手术开始后30 min(T3)、手术结束时(T4)患者的心率(HR)、平均动脉压(MAP)。测定血游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、甲状腺素(T4)、三碘甲状腺原氨酸(T3)、促甲状腺激素(TSH)等水平。 结果观察组T2、T3、T4点MAP、HR明显低于对照组(P<0.05)。观察组术后各时间点甲状腺素水平差异无显著性(P>0.05);观察组T3、T4点FT3、T3、TSH水平均明显高于对照组(P<0.05);观察组T2、T3、T4点FT4、T4水平均明显高于对照组(P<0.05)。观察组术后3、6、12、24hVAS评分均明显低于对照组(P<0.05)。观察组不良反应发生率显著低于对照组(χ2=6.157,P=0.017)。 结论全身麻醉复合颈浅丛阻滞麻醉下甲状腺术中甲状腺素水平比较稳定,且抑制术后躁动,具有较高的临床应用价值。

      To investigate the effect of general anesthesia combined with superficial cervical plexus block anesthesia on postoperative agitation and thyroid hormone level. Methods84 patients with thyroid surgery were randomly divided into two groups. The control group was given general anesthesia, and the observation group was given general anesthesia combined with superficial cervical plexus block anesthesia. Heart rate (HR) and mean arterial pressure (map) were compared before anesthesia induction (T0), 1 min after tracheal intubation (T1), skin incision (T2), 30 min after operation (T3) and at the end of operation (T4). The levels of free triiodothyronine (FT3), free thyroxine (FT4), thyroxine (T4), triiodothyronine (T3) and thyroid stimulating hormone (TSH) were measured. ResultsMAP and HR at T2, T3 and T4 points in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in thyroid hormone level at each time point after operation in the observation group (P>0.05); The levels of FT3, T3 and TSH at T3 and T4 in the observation group were significantly higher than those in the control group (P<0.05); The levels of FT4 and T4 at T2, T3 and T4 in the observation group were significantly higher than those in the control group (P<0.05). The VAS scores at 3,6, 12 and 24 hours after operation in the observation group were significantly lower than those in the control group (P<0.05). The incidence of adverse reactions in the observation group was significantly lower than that in the control group(χ2=6.157,P=0.017). ConclusionGeneral anesthesia combined with superficial cervical plexus block anesthesia thyroid surgery thyroxine level is relatively stable, and inhibit postoperative agitation, has high clinical application value.
