      目的探讨高频超声与磁共振成像(MRI)扫描对不同类型踝外侧韧带损伤的诊断价值。方法选取疑似踝外侧韧带损伤患者68例,进行高频超声及MRI检查,比较两种检查方法对不同类型踝关节外侧损伤、踝外侧韧带损伤分级、踝关节复合损伤的检出率。以手术结果为金标准,比较两种检查方法的灵敏度、特异度及准确率。结果MRI对踝外侧韧带损伤的检出率显著高于高频超声(P<0.05),其灵敏度为96.92%,特异度为100%,总体准确率为97.06%,Kappa值为0.735,一致性较高;高频超声对踝外侧韧带损伤的灵敏度为84.61%,特异度为100%,总体准率为85.29%,Kappa值0.327。在Ⅲ级损伤检查中,MRI检出率高于高频超声(P<0.05);复合损伤检查中,MRI对骨挫伤的检出率高于高频超声(P<0.05)。结论两种检查方法对不同类型踝外侧韧带损伤诊断效果均较好,但MRI略优于高频超声。
英文摘要:
      To explore diagnosis of high-frequency ultrasound and magnetic resonance imaging (MRI) scan for different types of ankle lateral ligament injury. MethodsA total of 68 patients suspected with ankle lateral ligament injury were enrolled. All underwent high-frequency ultrasound and MRI scan. The detection rates of the two methods for different types of lateral ankle injury, grading accuracy for ankle lateral ligament injury and detection for ankle composite injury were compared and observed. Taking surgical examination results as golden standard, sensitivity, specificity and overall coincidence rate of the two examination methods were observed. ResultsThe detection rate of MRI for ankle lateral ligament injury was significantly higher than that of high-frequency ultrasound (P<0.05). Sensitivity, specificity, overall coincidence rate and Kappa value of MRI for ankle lateral ligament injury were 96.92%, 100%, 97.06% and 0.735, respectively. High consistency, sensitivity, specificity, overall coincidence rate and Kappa value of high-frequency ultrasound for ankle lateral ligament injury were 84.61%, 100%, 85.29% and 0.327, respectively. The detection rate of MRI was higher than that of high-frequency ultrasound in grade III injury (P<0.05). In composite injury examination, detection rate of MRI for bone contusion was significantly higher than that of high-frequency ultrasound (P<0.05). ConclusionsBoth the two methods are of good diagnostic effect for different types of ankle lateral ligament injury. However, MRI examination is slightly better than high-frequency ultrasound.
