| 目的分析2型糖尿病合并肾周围炎的病原菌分布情况及危险因素。方法选取在本院就诊的2型糖尿病合并肾周围炎患者为合并肾周围炎组,2型糖尿病合并单纯尿路感染患者为尿路感染组；2型糖尿病不合并泌尿系感染患者为2型糖尿病组。对3组患者的全血、尿液、痰液进行病原菌培养及鉴定,并检测相关指标。结果合并肾周围炎组患者空腹血糖、餐后2 h血糖、C反应蛋白(CRP)、24 h尿蛋白定量水平均较高,糖化血红蛋白(HbAlc)水平较低；HbAlc、尿微量白蛋白、24 h尿蛋白定量是影响2型糖尿病患者合并肾周围炎的独立危险因素(P＜0.05)。结论在2型糖尿病合并肾周围炎患者的全血、尿液、痰液中病原菌以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌为主。HbAlc、尿微量白蛋白、24 h尿蛋白定量为2型糖尿病患者合并肾周围炎的重要危险因素。 |

| To study the distribution of pathogens in patients with type 2 diabetes mellitus complicated with perinephritis and analyze the risk factors. MethodsThe patients with type 2 diabetes mellitus complicated with perinephritis in our hospital were selected as the combined perinephritis group; the patients with type 2 diabetes mellitus complicated with simple urinary tract infection were selected as the simple urinary tract infection group; the patients with type 2 diabetes without urinary tract infection were selected as the type 2 diabetes group. The pathogens were cultured and identified, and the related indexes were detected. ResultsThe levels of fasting blood glucose, 2 h postprandial blood glucose, C-reactive protein (CRP) and 24-hour urinary protein in the combined perinephritis group were higher, while the level of glycosylated hemoglobin (HbAlc) was lower; HbAlc, urinary microalbumin and 24-hour urinary protein were the independent risk factors of type 2 diabetes mellitus complicated with perinephritis (P＜0.05). ConclusionEscherichia coli, Klebsiella pneumoniae and Pseudomonas aeruginosa were the main pathogens in the whole blood, urine and sputum of type 2 diabetic patients with perinephritis. HbAlc, microalbuminuria and 24-hour urinary protein are independent risk factors of perinephritis in patients with type 2 diabetes mellitus. |
