      目的探讨阿法替尼(Afatinib)体内外抗肺炎支原体(Mp)活性及其抗炎效果。方法通过颜色变化单位及最小抑菌质量浓度实验评价阿法替尼体外抑制Mp增殖作用。构建BALB/c小鼠肺炎支原体感染模型,经阿法替尼给药后,采集血液和肺组织标本,血细胞计数(白细胞总数、中性粒细胞、淋巴细胞、单核细胞)计算炎症指标;对肺组织标本进行Mp活菌计数;HE染色评价阿法替尼对小鼠Mp感染模型的抗炎效果。检测细胞内活性氧(ROS)水平及肺匀浆上清中肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)和白细胞介素-6(IL-6)含量。结果体外实验结果显示Mp组培养7天和14天的颜色变化单位分别为107 CCU/L、1010 CCU/L,而Afatinib组培养14天后仍无颜色变化,且阿法替尼最小抑菌质量浓度为16 mg/L。Afatinib组肺组织中活菌数量、ROS及TNF-α含量较Mp组明显降低(P<0.05),其白细胞总数、中性粒细胞数、炎症指数(NLR、MLR)亦显著低于Mp组(P<0.05),且肺组织切片炎症较Mp组明显减轻(P<0.05)。结论阿法替尼具有抑制肺炎支原体增殖的作用及抗炎效果,为临床上治疗肺炎支原体感染提供潜在的治疗依据。

      To explore the antibacterial and anti-inflammatory effect of Afatinib against mycoplasma pneumoniae (Mp) infection. MethodsThe color change unit (CCU) and the minimum inhibitory concentration were used to evaluate the effect of Afatinib in inhibiting the proliferation of Mp in vitro; Then, the model of mycoplasma pneumoniae infected BALB/c mice was constructed, and blood and lung samples after Afatinib treatment were collected. In this study, inflammation index were calculated by blood cell count (white blood cells, neutrophil , lymphocyte, monocyte); the effect of Afatinib on Mp proliferation in vivo was evaluated by the living Mp number in lung tissue; HE staining was used to estimate the anti-inflammatory effect of Afatinib on Mp infected mice. The level of intracellular reactive oxygen species (ROS) and tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and interleukin-6 (IL-6) in lung homogenate supernatant was detected to analyze its anti-inflammatory mechanism. Results The study showed that the color change units of the Mp group were 107 CCU/L and 1010 CCU/L after 7 and 14 days in vitro, while the Afatinib group had no color change after 14 days, and the minimum inhibitory concentration of Afatinib was 16 mg/L. The viable count, ROS and TNF-α in lung tissue of Afatinib group was significantly lower than Mp group (P<0.05), and the WBC, NEUT, inflammation index (NLR, MLR) were also significantly lower than Mp group (P<0.05), besides, the inflammation of lung slices was significantly relieved in comparision with Mp group. ConclusionAfatinib exhibits excellent antimicrobial and anti-inflammatory effects against Mp infection, which may provide a potential therapeutic basis in the treatment of Mp infection in clinical practice.
