目的研究血清microRNA-155(miR-155)联合白细胞介素-6(IL-6)水平检测对肝细胞肝癌(HCC)患者预后的评估价值。方法选取50例HCC患者,手术切除癌组织;另外选择同期50例体检健康者作为对照组。采用酶联免疫吸附试验(ELISA)检测及荧光定量PCR法检测两组IL-6及miR-155水平,比较两组miR-155、IL-6水平,ROC曲线分析IL-6联合miR-155水平与HCC患者预后的关系,比较其灵敏度、特异度及准确度,同时对HCC患者预后进行多元回归分析。结果HCC组IL-6、miR-155水平高于对照组(P<0.05)。IL-6=9.7 μg/L,miR-155=2.5为截断点,IL-6联合miR-155测定HCC患者预后的灵敏度、特异度及准确度均高于IL-6或miR-155单独测定(P<0.05)。TNM分期、肿瘤直径、病理分级、淋巴转移、门静脉侵犯、高水平miR-155、高水平IL-6是HCC患者预后的危险因素(P<0.05)。肿瘤大小、淋巴转移、门静脉侵犯、高水平miR-155及高水平IL-6是影响HCC患者预后的独立危险因素(P<0.05)。结论IL-6和miR-155联合检测可作为HCC诊断的指标之一,同时也可作为评估HCC患者预后的独立预测因子。 |

To evaluate the prognostic value of serum microRNA-155 (miR-155) combined with interleukin-6 (IL-6) in patients with HCC. Methods50 patients with HCC were selected and the carcinoma tissue was removed by operation. Another 50 healthy patients were selected as the control group. The levels of IL-6 and miR-155 in the two groups were detected by enzyme-linked immunosorbent assay (ELISA) and fluorescence quantitative PCR. The levels of miR-155 and IL-6 in the two groups were compared. The relationship between IL-6 combined miR-155 and prognosis of HCC patients was analyzed by ROC curve, and the sensitivity, specificity and accuracy were compared. ResultsThe levels of IL-6 and miR-155 in HCC group were higher than those in control group (P<0.05). IL-6=9.7 μg/L and miR-155=2.5 were cut-off points. The sensitivity, specificity and accuracy of IL-6 combined with miR-155 were higher than those of IL-6 or miR-155 alone (P<0.05). TNM stage, tumor diameter, pathological grade, lymph node metastasis, portal vein invasion, high level of miR-155 and high level of IL-6 were risk factors for the prognosis of HCC patients (P<0.05). Tumor size, lymph node metastasis, portal vein invasion, high level of miR-155 and high level of IL-6 were independent risk factors for the prognosis of HCC patients (P<0.05). ConclusionThe combined detection of IL-6 and miR-155 can be used as one of the indicators for the diagnosis of HCC, and as an independent predictor for the prognosis of HCC patients. |
