| 目的针对高血压伴焦虑抑郁的患者给予草酸艾司西酞普兰治疗,观察患者血压变异性的变化,旨在为临床治疗提供思路。 方法通过整群抽样法,回顾性分析65例高血压伴焦虑抑郁患者的临床资料。根据患者治疗方式不同进行分组,将给予高血压治疗联合心理干预治疗的32例患者纳入常规组；选择同时期在常规组治疗基础上给予草酸艾司西酞普兰治疗的33例患者纳入观察组。治疗3月后,比较两组治疗前、后患者24 h动态血压情况、负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]、生存质量综合评定问卷(GQOLI-74)、血清指标水平[神经元特异性烯醇化酶(NSE)、脑源性神经营养因子(BDNF)及钙结合蛋白B(S100B)]及治疗期间不良反应情况。 结果治疗后,观察组夜间收缩压标准差(nSSD)、白天舒张压标准差(dDSD)、夜间舒张压标准差(nDSD)、24 h舒张压标准差(24 hDSD)、白天收缩压标准差(dSSD)、24 h收缩压标准差(24 hSSD)均低于常规组(P＜0.05)；与常规组比较,观察组24 h动态血压水平、SAS、SDS评分均较低,GQOLI-74量表中躯体功能、物质生活、心理功能、社会功能各维度评分均较高(P＜0.05)；观察组NSE、S100B水平较低,BDNF水平较高(P＜0.05)；两组不良反应发生情况比较,差异无统计学意义(P＞0.05)。 结论高血压伴焦虑抑郁患者给予草酸艾司西酞普兰治疗,可有效缓解焦虑、抑郁状态,稳定血压水平,利于改善神经功能,提高生存质量,且安全性理想。 |

| Treating hypertension patients with anxiety and depression by escitalopram oxalate, to observe the changes of blood pressure variability in patients, aiming to provide ideas for clinical treatment. MethodsThe cluster sampling method was adopted, the clinical data of 65 hypertension patients with anxiety and depression were retrospectively analyzed. All patients were divided into groups according to different treatment methods. 32 patients who received hypertension treatment and mental intervention were included in routine group; 33 patients who received escitalopram oxalate on the basis of the routine group at the same period were included in observation group. After treatment for 3 months, the 24h ambulatory blood pressure, negative emotion [self-rating anxiety scale (SAS), self-rating depression scale (SDS)], general quality of life inventory (GQOLI-74), serum index levels [neuron specific enolase (NSE), brain-derived neurotrophic factor (BDNF) and calcium binding protein B (S100B)] and adverse reactions during treatment were compared before and after treatment. ResultsAfter treatment, the night systolic blood pressure standard deviation (nSSD), daytime diastolic blood pressure standard deviation (dDSD), night diastolic blood pressure standard deviation (nDSD), 24h diastolic blood pressure standard deviation (24 hDSD), daytime systolic blood pressure standard deviation (dSSD), 24h systolic blood pressure standard deviation (24 hSSD) in observation group were slightly lower than those in routine group (P＜0.05); compared with routine group, the 24h ambulatory blood pressure, SAS and SDS scores in the observation group were lower, and the scores of physical function, material life,mental function and social function of GQOLI-74 scale in the observation group were higher (P＜0.05); the levels of NSE and S100B in the observation group were lower, BDNF level was higher (P＜0.05); compared adverse reactions of both groups, there was no statistical significant difference (P＞0.05). ConclusionEscitalopram oxalate in treatment of hypertension patients with anxiety and depression can effectively relieve anxiety and depression, stabilize blood pressure level, which is beneficial to improving neurological function and quality of life with ideal safety. |
