目的探讨脑脊液气体分析与动脉瘤性蛛网膜下腔出血(aSAH)后迟发性脑缺血(DCI)的相关性。 方法选择165例aSAH患者,按是否并发DCI分为DCI组和非DCI组。分析比较两组一般临床资料和脑脊液检测结果。采用多因素Logistic多元回归模型对其影响因素进行分析。 结果165例aSAH患者中并发DCI 51例(30.9%)。与非DCI组比较,DCI组脑积液pH显著升高,而二氧化碳分压(PCO2)显著降低(P<0.05);两组间氧分压(PO2)、脑脊液碱剩余(BE)、血氧饱和度、总蛋白和炎性细胞等差异无显著性(P>0.05)。多因素Logistic多元回归分析显示,脑室铸型(OR=0.640)、基线GCS评分(OR=1.388)、Hunt-Hess分级(OR=2.145)、Fisher分级(OR=3.327)、脑脊液pH(OR=1.507)、脑脊液PCO2(OR=0.712)与DCI发生独立相关。 结论较高脑脊液pH和较低脑脊液PCO2是DCI潜在影响因素,可能是预测aSAH患者并发DCI的重要指标。 |

To investigate the correlation between cerebrospinal fluid gas analysis and delayed cerebral ischemia (DCI) after aneurysmal subarachnoid hemorrhage (aSAH). Methods165 patients with aSAH admitted to our hospital were divided into DCI group and non-DCI group according to whether patients were complicated with DCI. The general clinical data and cerebrospinal fluid test results of the patients were analyzed. Multivariate logistic regression model was used to analyze the related factors. ResultsIn 165 cases of aSAH patients, 51 cases were complicated with DCI, the incidence rate was 30.9%. Compared with non-DCI group, the CSF pH in DCI group was significantly increased, while the PCO2 was significantly decreased (P<0.05). There were no statistical difference between two groups in PO2, BE, SpO2, total protein and inflammatory cell (P>0.05). Multivariate Logistic regression analysis showed that ventricular casting (OR=0.640), baseline GCS score (OR=1.388), Hunt-Hess grade (OR=2.145), Fisher grade (OR=3.327), cerebrospinal fluid pH (OR=1.507) and cerebrospinal fluid PCO2 (OR=0.712) were independently associated with DCI. ConclusionHigher CSF pH and lower CSF PCO2 may be new potential factors related to the occurrence and progression of DCI, as well as an important index to predict DCI in patients with aSAH. |
