| 探讨闭合复位联合防旋股骨近端髓内钉(PFNA)和动力髋加压螺钉(DHS)在高龄股骨粗隆间骨折中的差异。选择不稳定型股骨粗隆间骨折患者,分为48例PFNA治疗(PFNA组)和40例DHS治疗(DHS组),比较两组患者手术效果。PFNA组手术时间、骨折愈合时间、术后负重时间、切口长度均短于DHS组,术中出血量少于DHS组,差异显著(P＜0.05)；PFNA组并发症发生率低于DHS组(12.50% vs 25.00%),差异显著(P＜0.05)；但两组患者在Harris评分及VAS评分比较均无显著差异(P＞0.05)；PFNA组在治疗结束时SF-36各维度评分均明显优于DHS组,差异显著(P＜0.05)。PFNA治疗高龄股骨粗隆间骨折优于DHS,值得推广和应用。 |

| To investigate the effects of closed reduction, proximal femoral nail (PFNA) and dynamic hip screw (DHS) on postoperative pain, limb function and quality of life in elderly patients with intertrochanteric fracture.Patients with unstable intertrochanteric fractures were selected, with 48 cases in group PFNA and 40 cases in group DHS, and the results of the two groups were compared. In PFNA group, operation time, fracture healing time, postoperative weight-bearing time, incision length were shorter than the DHS group, the amount of intraoperative bleeding was significantly less than that of group DHS (P<0.05); the complication rate of PFNA group was lower than that of DHS group (12.50% vs 25.00%), with significant difference (P<0.05); but the two groups of patients in the Harris and VAS scores showed no significant difference (P>0.05); PFNA group SF-36 the score at the end of treatment were significantly better than the DHS group, with significant difference (P<0.05).PFNA is superior to DHS in the treatment of intertrochanteric fracture in elderly patients, and is worthy of popularization and application. |
