      目的 观察丙种球蛋白联合氨溴索雾化吸入治疗婴幼儿重症肺炎的疗效。方法将116例患儿均分为治疗组与对照组各58例,均用氨溴索雾化吸入治疗,治疗组加用丙种球蛋白,一个疗程后比较两组总有效率、临床指标、肺功能、血清脑钠肽(BNP)和炎症因子水平差异。结果对照组总有效率74.1%,治疗组总有效率89.7%(P<0.05);治疗组呼吸改善、啰音吸收、咳嗽消失、住院时间均显著优于对照组(P<0.05);治疗后两组呼吸频率(RR)、每千克体重潮气量(TV/kg)、达峰时间比(TPTEF/TE)、达峰容积比(VPEF/VE)之间差异无显著性(P>0.05);治疗后治疗组心功能指标和炎症指标均显著低于对照组(P<0.05)。结论丙种球蛋白联合氨溴索雾化吸入治疗婴幼儿重症肺炎可显著改善心肺功能,提高治疗效果。

      Objective To explore the effect of gamma globulin combined with ambroxol inhalation in the treatment of severe pneumonia and cardiopulmonary function in infants.Methods116 infants with severe pneumonia were randomly divided into 2 groups,the control group was treated with ambroxol inhalation while the treatment group was treated with ambroxol inhalation combined with intravenous immunoglobulin.The total effective rate and clinical indicators were compared.Cardiopulmonary function,brain natriuretic peptide(BNP) and inflammatory factors were evaluated between both groups before and after treatment.ResultsThe total effective rate was 74.1% in the control group and 89.7% in treatment group,which was statistically significant (P<0.05).Respiratory improvement time,rales absorption time,cough disappeared time and hospitalization were significantly improved in treatment group than control group (P<0.05).The cardiopulmonary index RR,TV/kg,TPTEF/TE and VPEF/VE were improved in the both groups after treatment and there was no significant differences observed (P>0.05).The BNP,TNF-α and IL-6 level in the treatment group were significantly lower than those in the control group (P<0.05).ConclusionsThe combination therapy of immunoglobulin combined with ambroxol inhalation in the treatment of severe pneumonia infants can significantly improve the total therapy efficacy,cardiopulmonary function and inflammation.
