| 目的 探讨右美托咪定对利多卡因硬膜外阻滞时EC50的影响。方法在硬膜外阻滞下行下肢及下腹部择期手术的患者100例,年龄19~56岁,体重48~75kg,ASA分级Ⅰ~Ⅱ级,随机将其分为4组,每组25例,分别为利多卡因组,利多卡因混合右美托咪定(混合组)。采用序贯法测定各组利多卡因在硬膜外阻滞时皮肤钳夹反应的EC50,并记录Ramsay评分及不良反应的发生情况。结果利多卡因组的利多卡因EC50为1.29%~1.49%,混合组的利多卡因EC50分别为1.03%~1.14%,0.75%~0.84%,0.71%~0.81%,混合组与利多卡因组相比,EC50有统计学差异(P<0.05),Ramsay评分升高(P<0.05)；右美托咪定0.75 μg/kg混合组有2例患者出现窦性心动过缓。结论右美托咪定可降低利多卡因硬膜外阻滞时的EC50,并且在浓度0.50 μg/kg时达到最佳效果。 |

| Objective To investigate the effect of different dose dexmedetomidine on median effective concentration(EC50) of lidocaine for epidural block.Methods100 ASAⅠ~Ⅱpatients aged 19~56 years,with weight of 48~75 kg,scheduled for lower extremity or lower abdomen surgery under epidural block,were randomly divided into 4 groups(n=25 in each):lidocaine group,mixture groups(lidocaine combined with dexmedetomidine).Up-and-down sequential method was used to measure EC50 of lidocaine in each group.Ramsay score and adverse events were recorded.ResultsThe EC50 of lidocaine in lidocaine group were 1.29%~1.49%,in mixture groups were separately 1.03%~1.14%,0.75%~0.84%,0.71%~0.81%.Compared with lidocaine group,the EC50 of three mixture groups were all decreased significantly(P<0.05),the Ramsay scores were increased significantly(P<0.05).Two serious bradycardia happened in lidocaine combined with 0.75 μg/kg dexmedetomidine mixture group.ConclusionDexmedetomidine can significantly decrease the EC50 of lidocaine for epidural block,0.50 μg/kg dexmedetomidine can take the best effect. |
