| 目的 分析彩色多普勒超声对良性和恶性原发性卵巢肿瘤的临床辨别价值,为良恶性卵巢肿瘤的早期、无创鉴别诊断提供参考依据。方法以本院2010年3月~2015年3月收治的223例卵巢肿瘤患者为研究对象,回顾分析患者术前彩色多普勒超声检查与术后病理组织学检查结果,分析彩色多普勒超声诊断卵巢恶性肿瘤的灵敏度及特异性等指标。结果223例患者中,144例为良性卵巢肿瘤,占64.57%,其余79例为恶性卵巢肿瘤,占35.43%。良性卵巢肿瘤彩色多普勒超声血流分级显著低于恶性卵巢肿瘤(P＜0.05)；良性卵巢肿瘤血流阻力指数(RI)、血流搏动指数(PI)均显著高于恶性卵巢肿瘤(P＜0.01)。彩色多普勒超声诊断恶性卵巢肿瘤的灵敏度为87.34%,特异性为77.78%,约登指数为65.12%,阳性预测值为68.32%,阴性预测值为91.80%。结论良性和恶性原发性卵巢肿瘤在形态、血流分级、血流动力学参数方面存在明显差异,彩色多普勒超声可明确上述特点,在指导卵巢肿瘤良恶性辨别方面有重要参考价值。 |

| Objective To analyze the clinical value of color Doppler ultrasound in the diagnosis of benign and malignant ovarian tumors,and to provide a reference for the early and non-invasive diagnosis of the benign and malignant ovarian tumors.Methods223 patients with ovarian cancer were selected for study,which were treated in our hospital.The sensitivity and specificity of color Doppler ultrasound in the diagnosis of malignant ovarian tumors were analyzed,and postoperative histopathological examination were analyzed retrospectively.ResultsAmong the 223 cases,144 cases were benign ovarian tumors,accounting for 64.57%,and the other 79 cases were malignant ovarian tumors,accounting for 35.43%.The color Doppler ultrasound blood flow grading of Benign ovarian tumor was lower than that of malignant ovarian tumors,and the difference was statistically significant (P<0.05).The blood flow resistance index (RI) and the pulsatility index (PI) of benign ovarian tumor were higher than the malignant ovarian tumors,and the difference was statistically significant (P<0.01).The diagnostic sensitivity,the diagnostic specificity and the Youden index of color Doppler ultrasound for malignant ovarian tumor were 87.34%,77.78% and 65.12%,the positive predictive value and negative predictive value were 68.32% and 91.80%.ConclusionsThe morphology,flow classification,hemodynamic parameters had obvious difference between benign and malignant ovarian tumors,color Doppler ultrasound could clearly define these characteristics,which had important guidance value for the discrimination of benign and malignant ovarian tumor. |
