| 目的 探讨颈动脉支架成形术(CAS)治疗严重颈动脉狭窄合并2型糖尿病(T2DM)患者的效果及对患者凝血、炎症反应的影响。方法选取2013年3月～2015年5月本院进行CAS治疗的64例患者进行回顾性分析,将合并T2DM患者39例作为T2DM组、不合并糖尿病患者25例作为对照组,观察两组患者的治疗效果、凝血、炎症反应指标差异。结果两组的手术总有效率间差异无统计学意义(P>0.05)；两组术前的凝血酶原时间(PT)、活化部分活化凝血酶原时间(APTT)、纤维蛋白原(Fib)、D二聚体(DD)、血清超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)测定值间差异均无统计学意义(P>0.05)。术后3天,两组患者的PT、APTT、Fib、DD测定值差异均无统计学意义(P>0.05)；但T2DM患者的hs-CRP、TNF-α、IL-6、IL-8测定值显著高于对照组(P<0.05)。结论CAS治疗严重颈动脉狭窄合并T2DM患者的效果、及凝血功能影响与不合并糖尿病患者基本一致,但T2DM患者的炎症反应更加严重。 |

| Objective To investigate the effect of carotid artery stenting (CAS) as the treatment of carotid artery stenosis patients with type 2 diabetes mellitus (T2DM) and its influence on blood coagulation and inflammatory reaction.Methods64 patients accepted CAS teatment in our hospital from March 2013 to March 2015 were chosen for retrospective analysis.39 cases combined with T2DM were taken as T2DM group,25 cases without T2DM were taken as control group.The difference of therapeutic effect,coagulation and inflammation index between two groups was observed.ResultsTotal effective rate of operation in T2DM group and control group had no statistically significant difference(P>0.05);there was no significant difference between two groups on preoperative prothrombin time (PT),activated partial activation of prothrombin time (APTT),fibrinogen (FIB),D dimer (DD),serum high sensitivity C reactive protein (hs CRP) and the level of tumor necrosis factor alpha (TNF alpha),leukocyte mediated IL-6(IL-6) and interleukin IL-8 (IL-8) (P>0.05).The value of PT,APTT,Fib and DD in two groups of patients 3 days after operation also had no significant difference (P>0.05);but the levels of hs-CRP,TNF-α,IL-6 and IL-8 alpha in T2DM of patients were significantly higher than control group (P<0.05).ConclusionThe effect of CAS treatment on coagulation were consistented in severe carotid artery stenosis patients with T2DM whether combined with diabetes,but CAS treatment induced more severe inflammation in patients with T2DM. |
