| 目的 探讨高频震荡通气(HFOV)联合纳洛酮治疗新生儿窒息的临床疗效。方法以入院病例号为编号,根据随机数字表,将80例患儿回顾性分为对照组38例,观察组42例。对照组实施高频震荡通气治疗,观察组在对照组治疗的基础上联合纳洛酮治疗。观察两组患者治疗后疗效、主要症状恢复时间以及血气pH值、动脉氧分压(PO2)、吸入氧浓度(FiO2)的变化。结果两组患儿治疗后观察组疗效明显高于对照组(P＜0.01)。观察组患儿24 h内呼吸平稳、哭声有力、肌张力恢复、原始反射恢复率明显高于对照组。两组患儿随上机时间的延长pH值、FiO2值逐渐降低,PO2值逐渐升高；与对照组比较,观察组患儿上机1 h、8 h、24 h时pH值、FiO2值、 PO2值之间差异均有显著性(P＜0.05)。结论高频震荡通气(HFOV)联合纳洛酮治疗新生儿窒息方案疗效确切,较单纯高频震荡通气安全可靠,值得临床推广应用。 |

| Objective To investigate the high frequency oscillation ventilation combined with naloxone in treatment of asphyxia neonatorum.MethodsAccording to the admission number and random number table,80 cases were divided into the control group (38 cases) and observation group (42 cases).The control group were given high freguency oscillatory ventilation,the observation group were given naxolone on the basis of control group.The treatment efficacy,main symptoms recovery time and blood gas pH value,arterial oxygen tension,fraction of inspired oxygen of two groups after treatment were observed.ResultsThe efficacy of observation group was obviously higher than control group(P＜0.01);The breathe smoothly,cry strong,muscle tone recovery,primitive reflex recovery rate in 24 h of the observation group were obviously higher than the control group;The pH,FiO2 value of two groups were decreased,and the PO2 value of two groups were gradually raised with breathing machine prolonged.compared with the control group,the pH,FiO2,PO2 value of the observation group after using breathing machine for 1 h,8 h and 24 h were significantly different(P＜0.05).ConclusionHigh frequency oscillation ventilation combined with naloxone in treatment of asphyxia neonatorum had great efficacy,compared with high frequency oscillation ventilation,which was safte and worth of clinical promotion. |
