| 目的 探讨单侧输尿管上段嵌顿性结石患者的微创手术方案。方法选取2012年1月~2015年1月于本院就诊的单侧输尿管上段嵌顿性结石患者104例。将104例入选病例分为经皮肾镜碎石取石术(PCNL)组和后腹腔镜输尿管切开取石术(RLU)组,每组52例。回顾性分析两组患者手术时间、术中出血量、血红蛋白下降值、结石清除率、住院时间、住院费用和术后并发症的发生情况。结果RLU组术中出血量、手术时间、血红蛋白下降值、住院时间和住院费用显著低于PCNL组(P<0.05或P<0.01),两组结石清除率均为100%。PCNL组和RLU组术后并发症发生率分别为17.31%和19.23%,两组比较差异无显著性(P>0.05)。结论PCNL与RLU治疗输尿管上段嵌顿性结石均可取得良好的疗效,但RLU不损伤肾脏,出血少,适用于有条件开展RLU的医院开展。 |

| Objective To investigate the minimally invasive operation methods for unilateral incarcerate upper ureteric calculus.Method104 cases of patients with unilateral incarcerate upper ureteric calculus in our hospital from Jan 2011 to Jan 2015 were divided into percutaneous nephrolithotripsy (PCNL) group and retroperitoneal laparoscopic ureterolithotomy (RLU) group,52 cases in each group.The operation time,bleeding amount,hemoglobin declined value,clearance rate,hospital stays,hospital fees and postoperative complications were observed.ResultsThe operation time,bleeding amount,hemoglobin declined value,hospital stays,hospital fees of RLU group were significantly lower than that of PCNL group (P<0.05 or P<0.01).The clearance rate of both group was 100%.The postoperative complications occurrence rate of PCNL group and RLU group were 17.31% and 19.23%,respectively,without significant difference (P>0.05).ConclusionPCNL and RLU treatment on unilateral incarcerate upper ureteric calculus both could show good effect.However,RLU did not exert damage on kidney,with low bleeding amount and was adaptable for capable hospitals. |
