目的 探讨低分子肝素钙(LMWH)治疗慢性阻塞性肺疾病(COPD)引起的肺心病急性加重期患者的临床效果及对患者肺动脉压(PASP)的影响。方法106例继发于COPD的肺心病急性加重期患者随机分为试验组和对照组,两组各53例,并均给予常规吸氧、抗感染、强心等基础治疗,试验组患者同时予以LMWH治疗,对比两组治疗效果及PASP的变化。结果治疗前,两组按纽约心脏病协会分级标准(NYHA)心功能分布差异无统计学意义(P>0.05)。治疗后,与对照组患者比较,试验组的6min步行距离显著增加,NYHA心功能分级也显著优于对照组(P均<0.05);全血黏度高切、全血黏度低切、红细胞聚集指数显著降低(P<0.05);pH、PaO2显著升高,但PaCO2,PASP显著降低(P均<0.05);临床疗效分布显著改善(P<0.05)。结论 LMWH结合常规疗肺心病急性加重期患者能够提高临床效果,显著的改善患者的肺动脉压。 |

Objective To investigate the clinical effect of low molecular weight heparin calcium(LMWH) in patients with chronic obstructive pulmonary disease(COPD) and its effect on pulmonary arterial pressure(PASP) in patients with acute exacerbation of chronic pulmonary heart disease.Methods106 cases of acute pulmonary heart disease secondary to the exacerbation of COPD patients were divided into experimental and control group,53 cases were even assigned to the two groups,and cured by conventional oxygen,antibiotics,cardiac and other basic treatment, the test group received LMWH treatment simultaneously,and two groups of treatment and change of PASP was compared.ResultsBefore treatment,NYHA functional distribution showed no significant difference between the two groups.After treatment,compared to the control group,the test group had significantly improved heart functional grade,and the 6 min walking distance was also longer (P<0.05);the whole blood viscosity high shear,low shear whole blood viscosity, erythrocyte aggregation index was significantly lower than the control group(P<0.05);after treatment in the test group pH and PaO2 were significantly higher than control group(P<0.05), but PaCO2 and PASP were significantly lower than the control group(P<0.05);after treatment the clinical efficacy trial distribution was significantly better than the control group(P<0.05).ConclusionLMWH therapy combined with conventional therapy in patients with acute exacerbation of pulmonary heart disease can improve clinical results,with a significant improvement in pulmonary artery pressure in patients. |
