| 目的 比较血清降钙素原(PCT)、C反应蛋白(CRP)与白细胞(WBC)对颅脑手术患者发热的诊断效果,为临床选择合适的诊断指标提供理论依据。方法选取120例颅脑手术发热患者,按照患者有无感染将患者分为感染组和未感染组,每组患者60例。对两组患者的PCT、CRP以及WBC水平进行测定,比较三者对颅脑手术发热患者感染的诊断价值。结果感染组患者的PCT、CRP、WBC水平分别为1.25±0.58 μg/L、61.37±22.78 mg/L、(16.09±4.88)×109/L,明显高于未感染组患者0.15±0.12 μg/L、27.11±16.60 mg/L、(10.66±4.13)×109/L(P<0.05)；PCT、CRP、WBC诊断颅脑手术发热患者感染的灵敏度分别为96.11%、88.67%、86.95%,特异度分别为90.23%、70.62%、62.16%,PCT的灵敏度与特异度明显高于CRP与WBC(P<0.05)。结论PCT诊断颅脑手术发热患者感染时,具有明显高于CRP和WBC的灵敏度与特异度,建议作为临床诊断的主要炎症指标。 |

| Objective To compare the effects of WBC,CRP and WBC on diagnosing brain surgery patients with fever,and provide a reasonable basis for clinical choose.Methods120 cases of brain surgery patients with fever from January 2014 to June 2015 in our hospital were chosen for the study,and all the patients were divided into infection group and uninfected group according to infected or not,60 cases of patients in each group.PCT,CRP and WBC levels of two groups were measured to compare effects of WBC,CRP and WBC on diagnosing brain surgery patients with fever.ResultsPCT,CRP,WBC levels of patients in infection group were (1.25±0.58) μg/L,(61.37±22.78) mg/L,(16.09±4.88)×10-9/L respectively,which were significantly higher than non-infected patients (0.15±0.12) μg/L,(27.11±16.60) mg/L,(10.66±4.13) ×109-9/L,the difference was statistically significant (P<0.05); the sensitivity of PCT,CRP,WBC in diagnosing brain surgery patients with fever were 96.11%,88.67%,86.95% respectively,and the specificity were 90.23%,70.62%,62.16% respectively,the sensitivity and specificity of PCT were significantly higher than WBC and CRP,and the difference was statistically significant (P<0.05).ConclusionPCT has significant higher sensitivity and specificity than WBC and CRP in diagnosing brain surgery patients with fever,which is recommended as a major inflammatory markers of clinical diagnosis. |
