      目的观察新生儿缺氧性肺动脉高压(HPH)患儿血浆N端脑钠肽前体(NT-proBNP)的变化特点以及与HPH的相关性。方法围产期缺氧性新生儿根据生后第一天是否合并肺动脉高压分为HPH组与非HPH组。HPH组依据其生后第7天肺动脉压力(PASP)是否恢复正常分为HPH恢复正常组和HPH未恢复正常组。选择同期入院无围产期缺氧病史的新生儿为对照组。所有对象于生后第1天、第7天采集静脉血,测定其血浆NT-proBNP水平。结果生后第一天各组血浆NT-proBNP比较,HPH组与非HPH组NT-proBNP均高于对照组(P<0.05);中度及重度HPH组高于非HPH组和轻度HPH组(P<0.05);重度HPH组高于中度HPH组(P<0.05)。生后第7天HPH恢复正常组及未恢复正常组血浆NT-proBNP与第一天比较,血浆NT-proBNP均低于第一天(P<0.05)。生后第一天血浆NT-proBNP与PASP呈正相关(r=0.826,P=0.000)。结论血浆NT-proBNP水平与HPH患儿心功能状态及PASP高低密切相关,动态检测血浆NT-proBNP水平,可反映其疾病转归。

      ObjectiveTo determine the variability of plasma NT-proBNP and its value in the diagnosis of right ventricular function in infants with hypoxic pulmonary.MethodsNewborns with perinatal hypoxia were enrolled in this study,they were classified into the hypoxic pulmonary hypertension (HPH)group and the non HPH group within the first day of life.At the seventh day of life,the HPH group were divided into recovered group and non recovered group according to whether the pulmonary artery systolic pressure (PASP) came to normal or not.Newborns without perinatal hypoxia served as control group in the same period.Plasma NT-proBNP of the newborns were measured at the first day,seventh day after birth.ResultsAt the first day of life,newborns in the HPH and non HPH group had a significantly higher plasma level of NT-proBNP than newborns in the control group(P<0.05).Also,newborns in the moderate and severe group had a higher plasma level of NT-proBNP than newborns in the mild group(P<0.05).There was a significant reduction in the NT-proBNP levels at seventh day than that of the first day of life in the HPH group(P<0.05).There was a positive correlation between NT-proBNP levels and PASP at the first day(r=0.826,P=0.000).ConclusionThe high plasma level of NT-proBNP in newborns with HPH correlated with the cardiac function and the PASP.Serial monitoring of NT-proBNP may be helpful to reflect the status of the diseases.
