| 目的探讨阿德福韦酯治疗慢性乙肝患者HBsAg水平疗效的临床价值。方法化学发光法检测40例慢性乙型肝炎患者在阿德福韦酯治疗前和治疗过程中HBsAg水平，聚合酶链反应技术检测HBV DNA及连续监测法检测ALT水平，并分析HBV DNA与HBsAg的相关性。结果40例乙肝患者治疗前HBV DNA水平为（7.12±0.97）×106 IU/mL,HBsAg水平为（1.91±0.37）×103 IU/mL；经阿德福韦酯治疗3个月HBsAg已下降，6个月后显著下降（P<0.05），治疗3个月后HBV DNA显著下降（P<0.05）；治疗两年后HBsAg和HBV DNA降至较低水平即（0.86±0.77）×103 IU/mL、（3.12±0.72）×102 IU/mL；治疗3个月ALT显著下降，治疗6个月基本恢复正常；治疗两年后其中1例慢性乙型肝炎患者产生耐药，从而导致HBV DNA、HBsAg和ALT水平反弹；同时HBsAg与HBV DNA之间呈正相关。结论定期检测HBsAg浓度，有助于评估阿德福韦酯治疗乙肝疗效和及时发现耐药。 |

| ObjectiveTo investigate the clinical value of HBsAg in the assessment of efficacy of the chronic hepatitis B patients with adefovir dipivoxil treatment.MethodsIn serum of 40 cases of chronic hepatitis B patients with adefovir dipivoxil treatment HBsAg level was detected by Chemiluminescence,HBV DNA was detected by Polymerase chain reaction and Alanine aminotransferase was detected by continuous monitoring method,at the same time correlation between HBV DNA and HBsAg was analysed.ResultsIn 40 cases of hepatitis B patients pre-treatment HBV DNA level was （7.12±0.97）×106 IU / mL and HBsAg level was （1.91±0.37）×103 IU / mL;after three months of adefovir dipivoxil treatment,HBsAg declined,and after six months it was significantly decreased (P<0.05)，after 3 months HBV DNA was significantly decreased (P<0.05);one year later HBsAg was （0.86±0.77）× 103 IU / mL and HBV DNA was （3.12±0.72）×102 IU/mL;Alanine aminotransferase was significantly decreased after three months of treatment,and was about normal after 6 months of treatment;two years later in one chronic hepatitis B patient HBV DNA and HBsAg levels were greatly increased,and there was a positive correlation between HBsAg and HBV DNA.ConclusionPeriodic testing of HBsAg concentration could evaluate efficacy of hepatitis B patients with adefovir dipiv-oxil treatment and discover resistance timely. |
