目的总结应用微创锁定钢板固定股骨远端AO-C型骨折的疗效。方法回顾性分析采用微创锁定钢板固定股骨远端C型骨折46例患者的完整资料。骨折按AO分型:C1型6例,C2型22例,C3型18例。开放性骨折12例。手术方式为:直视下复位并临时固定股骨髁部,采用间接复位技术以锁定钢板桥接固定于股骨干。结果平均手术时间96 min,平均失血量284 mL。随访时间12~54个月,平均24.8个月。股骨远端骨折均愈合,平均愈合时间4.5个月(3~6个月)。7例发生并发症。膝关节功能按HSS评分标准评定:优26例,良18例,中2例,优良率95.6%。结论微创锁定钢板固定股骨远端C型骨折,具有手术创伤小、固定可靠及关节功能恢复好的优点,适用于股骨远端C型骨折中各亚型骨折的固定。 |

ObjectiveTo evaluate the effect of minimally invasive locked plate (MILP) in treating distal femoral AO type C fractures.MethodsFrom June 2005 to June 2010,46 patients with distal femoral AO type C fractures were treated with MILP.According to AO classification,there were 6 cases of type C1,22 cases of type C2 and 18 cases of type C3.Twelve cases were open fractures.The condyle fracture was visibly reduced and stabilized by temporary fixation,and then it was fixed to femoral shaft by the technique of indirect replacement and bridging osteosynthesis.ResultsThe average operation time was 96 minutes.The average blood loss was 284 mL.Forty-six patients were followed up 12 to 54 months (average 24.8 months).All distal femoral fractures healed and the average healing time were 4.5 months (3~6 months).Seven complications were observed in this study.According to the HSS criterion for evaluation,the results were excellent in 26 cases,good in 18 cases and fair in 2 case,the excellent and good rate was 95.6%.ConclusionMILP has the advantages of micro invasion,stable fixation and good function in the treatment of distal femoral AO type C fractures,and it can be used in any subtypes of these fractures. |
