      目的探讨氯沙坦钾对慢性心力衰竭患者尿液水通道蛋白2(AQP2)浓度的影响。方法慢性心力衰竭(CHF)患者203例,分为常规治疗组(100例)和氯沙坦钾组(103例),另设正常对照组(50例),用放射免疫法检测血浆血管加压素(AVP)浓度、间接ELISA法检测尿液AQP2浓度,用药12周,治疗前后各1天采血检测血浆AVP浓度、留尿检测尿液AQP2浓度,比较其变化。结果与正常对照组比较,氯沙坦钾组治疗前后AVP和AQP2浓度均显著升高(P均<0.05),常规治疗组治疗前后AVP和AQP2浓度均显著升高(P均<0.05)。氯沙坦钾组治疗后AVP和AQP2浓度低于药物治疗前,也低于常规组治疗后(P均<0.05)。结论CHF患者尿液AQP2排出增多,氯沙坦钾减少尿液AQP2排出,抑制集合管对水的重吸收,减轻水钠潴留,减轻心脏前负荷,延缓心力衰竭发展。

      ObjectiveTo investigate effects of losartan potassium on the concentration of urine aquaporin-2 (u-AQP2) in congestive heart failure patients.Methods203 patients with CHF were randomly divided into two groups: losartan potassium group (103 cases) and general treatment group (100 cases).These patients were compared against 50 normal control subjects.In all subjects,the concentration of plasma arginine vasopressin (p-AVP) were detected by radioimmunoassay (RIA),the concentration of u-AQP2 were detected by indirect enzyme linked immunosorbent assay (indirect ELISA),to analyse the relationship of u-AQP2 and p-AVP before and after 12 week treatment.ResultThe concentration of p-AVP and u-AQP2 in losartan potassium group and general treatment group were increased compared with normal control subjects before and after treatment(P<0.05); the concentration of p-AVP and u-AQP2 in losartan potassium group were decreased after treatment(P<0.05),and were decreased compared with general treatment group after treatment(P<0.05).ConclusionThe Losartan potassium downregulated renal AQP2 gene and protein expression,reduced the increased excretion of the u-AQP2 in CHF patients, reduced cardiac preload and delayed the development of CHF by blocking the water reabsorption in collecting duct.
