| 目的观察急性期颅脑损伤患者血清P-选择素及D-二聚体的变化规律，分析其与病情严重程度及预后的相关性。方法急性颅脑损伤患者96例，按斯堪的纳维亚卒中量表（SSS）进行神经功能缺损评分，在第1、3、30天检测血清P-选择素、D-二聚体、血糖水平及SSS评分。P-选择素、D-二聚体浓度采用酶联免疫吸附法(ELISA)检测。并对P-选择素、D-二聚体与SSS评分进行相关性分析。以同期20例健康体检者作为对照组。结果急性颅脑损伤患者依据SSS评分将患者分为轻、中、重型，P-选择素及D-二聚体水平均随分型加重而明显增加（P<0.05）；颅脑损伤患者各时间血糖、P-选择素及D-二聚体、SSS评分水平与对照组比较差异有显著性（P均<0.05）；血清P-选择素及D-二聚体水平与SSS下降值呈直线正相关（r=0.513，P<0.05）；Logistic回归分析显示，血清P-选择素及D-二聚体水平为急性颅脑损伤患者预后的独立危险因素及预后的有效指标。结论急性颅脑损伤患者血清P-选择素及D-二聚体水平可以作为判断病情和预后的重要指标。 |

| ObjectiveTo observe the associability about the levels of P-selectin and D-dimer, degree of disease and prognosis of patients with acute brain injury.Methods96 patients with acute brain injury were divided into three groups according to SSS. P-selectin and D-dimer in plasma were measured by ELISA. P-selectin and D-dimer in plasma，blood glucose and SSS score were measured in day 1,3,30 after brain injury. Correlation of the P-selectin，D-dimer and SSS score was explored using Logistic analysis. 20 healthy persons were enrolled into the control group.ResultsP- selectin and D-dimer levels of light, medium and heavy patients on admission were gradually increased .The levels of P-selectin，D-dimer，blood glucose and SSS score were higher in acute brain injury group than that in control group(P< 0.05) and were positively correlated with SSS score（r=0.513，P<0.05）. Logistic regression analysis showed that P-selectin，D-dimer might be potential prognostic factors for patients with acute brain injury．ConclusionThe levels of Plasma P-selectin and D-dimer can be the important index judging condition and the prognosis in patients with acute brain injury |
