| 目的评价热碘油经导管肝动脉化疗栓塞(HTACE)联合冷循环射频消融术(CRFA)治疗肝细胞癌(HCC)的临床价值。方法62例HCC分为单纯经导管肝动脉化疗栓塞(TACE)治疗组30例，HTACE联合CRFA治疗组32例。比较治疗前后影像学检查及血清甲胎蛋白(AFP)、丙氨酸氨基转移酶（ALT）并评价疗效。结果TACE（联合组）组的肿瘤显著坏死率、缩小率、血清AFP下降率、肿瘤复发转移率、肿瘤进展率、患者2年生存率分别为40.0%、46.7%、30.0%、36.7%、63.3%、20.0%,联合组分别为75.0%、71.9%、75.0%、12.5%、15.6%、46.9%,两组比较差异均有显著性(P<0.05)；两组ALT的好转率、无变化率及恶化率分别为20.0%、56.7%、23.3%和15.6%、46.9%、37.5%,两组比较差异均无显著性(P>0.05)。结论HTACE联合CRFA治疗肝细胞癌效果好、安全可靠、优势互补，疗效优于单纯TACE术，且具有很好的应用价值和前景。 |

| ObjectiveTo evaluate clinical value of hot lipiodol transcatheter hepatic arterial chemoembolization(HTACE) combined with cool-tip radiofrequency ablation(CRFA) on treatment of hepatocellular carcinoma.Methods62 cases of HCC were divided into the 30 cases of TACE treatment and the 32 cases of HTACE combined with CRFA treatment.The curative effect was evaluated by comparing before and after treatment of imaging examination and serum alpha-fetoprotein (AFP),alanine aminotransferase(ALT).ResultsThe tumor necrosis rate,significantly reducing rate,decline rate of AFP,recurrence rate,tumor progression rate,2 year survival rate were respectively 40.0%,46.7%,30.0%,36.7%,63.3%,20.0% in the TACE treatment group,and 75.0%、71.9%、75.0%、12.5%、15.6%、46.9% in the HTACE combined with CRFA group,which were all statistically different by comparison (P<0.05).The two groups ALT recovery rate,no change rate and deteriorating rate were respectively 20.0%,56.7%,23.3% and 15.6%,46.9%,37.5%,which were not statistically different by comparison (P>0.05).ConclusionHTACE combined with CRFA in treatment of hepatocellular carcinoma had good effect,complementary advantage,safety and reliability.The curative effect was better than the TACE.So it had good application value and prospect. |
