| 目的观察与常规氯胺酮肌肉注射麻醉比较，七氟醚吸入诱导麻醉对降低围术期儿童及其家属焦虑水平的影响。方法择期ASA Ⅰ～Ⅱ级，年龄1～ 3岁唇腭裂整复术患儿80例，随机分为实验组(n=42，七氟醚吸入麻醉)和对照组(n=38，常规氯胺酮肌肉注射麻醉)。采用改良耶鲁围术期焦虑量表(mYPAS)于术前访视（T1）、入手术室（T2）、诱导时（T3）、术后24 h（T4）4个时间点测量儿童的焦虑值，以诱导期合作评分(ICC)观察诱导时患儿合作程度,以Zung氏焦虑状态自测量表（SAS）测量患儿亲属的焦虑程度。结果与T1时间点比较，两组患儿T2及T3两个时间点焦虑评分明显升高（P<0.01）；与对照组比较，实验组患儿在T3时其焦虑评分明显降低（P<0.01），同时合作评分也明显降低（P<0.01）。实验组患儿亲属围术期焦虑程度（SAS）评分无明显变化（P>0.05）;而与T1时间点比较，对照组患儿亲属的焦虑程度（SAS）评分T2时点明显升高（P<0.01）,T3回到术前值,且与实验组比较T2时间点明显升高（P<0.01）;两组患儿家属对医务人员满意率差异无显著性。结论比较于常规氯胺酮肌肉注射麻醉，七氟醚吸入诱导麻醉可以明显降低婴幼儿及其亲属围术期的焦虑水平。 |

| ObjectiveTo investigate perioperative anxiety of induction with inhalation of sevoflurane and ketamine intramuscular injecton anesthesia in children and their parents.Methods80 cases with cheilopalatognathus were divided into two groups randomly:induction with inhalation of 8%sevoflurane(S group)or ketamine intrarmuscular injection anesthesia(C group).The anxiety level were tested by mYPAS(modified Yale preoperative anxiety scale)at 4 time points：the day before operation,into the operating room,anesthesia induction and 24 hours after operation.The cooperative levels during the anesthesia induction were tested by ICC(induction compliance checklist).ResultsThere were significantly higher anxiety level during into the operating room and the anesthesia induction than that the day before operation,but the children in S group showed lower anxiety level and better cooperation than that in C group during the anesthesia induction.The changes of Self-rating Anxiety Scale in S group decreased significantly than the C group.ConclusionCompared to ketamine intramuscular injection anesthesia,induction with inhalation of 8%sevoflurane could have a good effect to lower the anxiety level of the children and their parents. |
