| 目的观察B族维生素加地西泮对老年髋关节置换患者术后谵妄的疗效。方法76例患者随机分为治疗组(40例)和对照组(36例)，采用精神错乱鉴定法（CAM）进行评估，对谵妄者再测评谵妄严重程度（DRS），记录首、末次DRS评分和谵妄持续时间。治疗组采用地西泮每千克体重维生素B1 2 mg/日和维生素B12 10 μg/日（分2次肌注）以及维生素B6 8 mg/日静滴。对照组除不用上述药物外其它治疗与治疗组一致。观察两组患者谵妄发生率、谵妄严重程度、谵妄持续时间、年龄对术后谵妄的影响。结果治疗组和对照组谵妄发生分别为6例(15%)和27例(75%)；谵妄程度DRS首、末次评分分别为17.43±3.32、8.08±2.84和20.68±4.28、11.05±3.31；谵妄持续时间分别为2.04±1.04天和3.36±1.56天(P<0.05)。结论B族维生素加地西泮可能会减少和减轻老年髋关节置换患者术后谵妄的发生。 |

| ObjectiveTo observe the outcomes of premedication with VitBn and nitrazepam on postoperative delirium of eldly patients with hip replacements.Method76 old patients in my hospital were randomly divided into two groups:management group(40 cases)and conventional orthopedic management(control)group(36cases).A nitrazepam were injected in a dose of 0.2 mg each day and everyone,vitB12 mg and vitB6 8 mg with vitB12 10 μg in two dose each day.Daily assessment was based on patient interview with the CAM.DRS was used to measure symptoms during the prodromal phase after the onset of delirium.The scores of DRS by first and last assessment and the duration of delirium,the influence of ages on postoperative delirium were recorded.ResultThe mean length of hospital stay of the patient receiving management was 12.80±2.15 and that of the control was 13.73±2.12;a proportion of patients receiving management was 15 percents(6 cases),and that of the control was 75 percents(27cases);the first DRS scores，delirium duration and the last DRS scores respectively were 17.43±3.32，8.08±2.84，2.04±1.04;in the controls respectively were 20.68±4.28，11.05±3.31，3.36±1.56(respectively，P<0.05).ConclosionVitBn and nitrazepam can reduce the incidence of postoperative delirium of eldly patients with hip replacements. |
