| 目的 比较腹水超滤浓缩回输与传统方法治疗肝硬化难治性腹水的疗效、并发症、住院时间及住院费用的差别.方法 选择本院住院的此类患者58例,随机分为治疗组与对照组.治疗组32例,应用WLF-500型伟力腹水超滤回输系统共进行49次治疗;对照组采用传统的限水、限钠、利尿、输注白蛋白、血浆等治疗.观察记录治疗前后患者症状、体征、肝肾功能、并发症、住院时间和费用.结果 (1)治疗组临床症状缓解率明显高于对照组,两组差异有显著性(P＜0.05).(2)体征:治疗组腹围下降明显多于对照组,两组差异有显著性(P＜0.05).(3)肝功能:两组胆红素、转氨酶治疗后无明显变化(P＞0.05),但总蛋白、白蛋白均有提高,治疗组明显高于对照组,两组差异有显著性(P＜0.05).(4)肾功能:治疗组肌酐水平下降,对照组无明显变化,两组差异有显著性(P＜0.05).(5)副作用:治疗组并发腹膜炎2例,对照组出现电解质紊乱5例、肝性脑病2例、肝肾综合征1例,两组差异有显著性(P＜0.05).(6)平均住院日与费用:治疗组平均住院日为10.5±5.2天,对照组为16.2±4.6天,两组差异有显著性(P＜0.05);治疗组平均住院费用3080.5±620.6元,对照组组为4042.4±520.8元,两组差异有显著性(P＜0.05).结论 腹水超滤浓缩回输治疗难治性腹水较传统的方法更有效、安全、省时、省钱. |

| Objective To investigate the therapy effect of obstinate ascites by ultrafilter condensation importation. Methods 58 cases were randomly allocated to therapy group(32) and control group(26).Therapy group was treated with ultrafilter condensation importation by WLF-500,49 times.Control group was treated with dipsotherapy,sodium restriction,diuretic therapy,complement of protein or blood plasma.Control symptom,physical sign,liver and renal function,complication,hospitalization time and money were observed. Results(1)Symptom remission rate is better therapy group than control group(P<0.05).(2)Physical sign,all the circumference of abdomen were descent,therapy group and control group have significant difference(P<0.05).(3)Liver function:the bilirubin and transpeptidase have no difference(P>0.05),but albumin and total protein of therapy group were higher than control group(P<0.05).(4)Renal function:creatinine of therapy group were decreased,but control group's Enema didn't change(P<0.05).(5)Side effect: therapy group had 2 cases of peritonitis;control group had electrolyte 5 cases of disturbance,2 cases of hepatic encephalopathy, 1 cases of hepatorenal syndrome.(6)Hospitalization time and money:therapy group,10.5 5.2 day,3 080.5 620.6 yuan,control group,16.2 4.6,4 042.4 520.8 yuan(P<0.05).Conclusion The therapy effect of ultrafilter condensation importation is better than control group.It's more efective,it can shorten hospitalization time and money. |
