| 目的 探讨早期使用大剂量丙种球蛋白(IVIG)联合甲强龙治疗重症病毒性脑炎(SVE)的疗效.方法 对70例重症病毒性脑炎患者随机分为3组:常规组23例按照常规方法治疗;IVIG组20例在常规治疗的基础上在入院3天内给予IVIG静脉注射;IVIG 甲强龙组27例在IVIG组治疗的基础上在入院当天加甲强龙静脉滴注,测定3组治疗前后血浆白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,观察3组的临床疗效.结果 IVIG 甲强龙组发热、昏迷、抽搐、肢瘫持续的时间及平均住院时间明显短于常规组和IVIG组(P＜0.05);IVIG 甲强龙组呼吸衰竭、消化道出血、感染性休克、院内感染发生率及病死率较常规组明显降低(P＜0.05);IVIG 甲强龙组与IVIG组比较上述临床症状持续的时间、平均住院时间及呼吸衰竭和病死率比较差异均有显著性(P＜0.05).IVIG 甲强龙组、IVIG组和常规组IL-6和,TNF-α水平依次降低(P＜0.05).结论 早期应用大剂量丙种球蛋白联合甲强龙治疗重症病毒性脑炎能改善临床症状,缩短病程,降低其并发症的发生率,且病死率明显下降. |

| Objective To explore the clioucal efficiacy of treating severe viral encephalitis by IVIG unite MP in earlier period. Methods 70 severe viral encephalitis patients were divided into 3 groups randomly: under routine therapy,23 patients were treated with combined therapy. On the base of rountine therapy,20 patients were added with IVIG,the way of IVIG was added in encephalitis.The IVIG MP group(27 patients) added MP on the base of the IVIG group on the day of being in hospital.Curative efficiency of the 3 groups were observed.The expression levels of IL-6,TNF-a of the 3 groups before and after the therapy were detected and compared with each other.Results The lasting time of the fever,coma,compulsion limbs paralysis and the average stay in IVIG MP group is significantly shorter than the routine group and IVIG group(P<0.05).The case fatality rate and the incidence rate of respiratory failure,alimentary tract hemonnhage,infections shock,hospital onset of infection is significantly lower in IVIG MP group than that in routine group(P<0.05).The significant difference were also between IVIG MP group and IVIG group in lasting time of above-metioned clinical symptom,average stay and respiratory failure,case fatality rate(P<0.05).Conclusions Large dose of in travenous immunoglobulin(IVIG) MP in earlier period to treat severe viral encephalitis can improve clinical sympton,shorter the course of disease and decrease concurrent disease and the case fatality rate is also reduced significantly. |
