      目的 探讨白介素8(IL-8)和白介素10(IL-10)在慢性阻塞性肺病(COPD)血清中的表达及阿奇霉素对其的影响.方法 COPD急性加重期住院患者50例,随机分为常规治疗组(25例)和阿奇霉素治疗组(25例).另取10例健康者为正常对照组.分别采集入院时、治疗后第8天外周血,取血清用ELISA测定IL-8和IL-10水平,观察两组临床疗效及肺功能.结果 常规治疗组有效率68%,阿奇霉素组有效率92%,两者差异有显著性(P<0.05).治疗后阿奇霉素组FEV1.0、FEV1.0/FVC均高于常规治疗组,差异有显著性(P<0.01).治疗后,阿奇霉素组IL-8低于常规治疗组,差异无显著性;IL-10高于常规治疗组,差异有显著性(P<0.05).结论 阿奇霉素可能通过启动机体免疫机制,释放炎症抑制因子,抑制气道炎症反应.

      Objective To discuss the expression of Interleulkin-8(IL-8) and Interleulkin-10(IL-10) in the serum of patients with chronic obstructive pulmonary disease(COPD) and the affection of Azithromycin.Method We choose 50 acute and aggravated inpatients with chronic obstructive pulmonary disease and the affection of Azithromycin,who were divided into routine therapy group(25 cases) and Azithromycin therapy group(25 cases) randomly.In addition 10 healthy volunteers were chosen as control group.We examine the pulmonary function and detect the level of Interleulkin-8 and Interleulkin-10 with serum of peripheral blood by ELISA when they come to hospital and 8 days later,and then compare the curative effect and pulmonary function of the two therapy groups.Result The actovoty ratio of routine therapy group and Azithromycin therapy group is 68% and 92% respectively,there is significant difference(P<0.05).FEV1.0 and FEV1.0/ FEV of Azithromycin therapy group is higher than routine therapy group significantly(P<0.05).The level of nterleulkin-8 of Azithromycin therapy group is lower than routine therapy group,but there is not significant difference(P>0.05).The level of Interleulkin-10 of Azithromycin therapy group is higher than routine therapy group significantly(P<0.05).Conclusion It is possible that Azithromycin release inflammatory mediator and restrain respiratory inflammatory response by initiating body immune mechanism.
