| 目的 采用乳胶凝集法和速率散射比浊法检测类风湿因子(RF),比较两种方法对类风湿性关节炎(RA)的诊断价值,得出本实验室使用比浊法对RA诊断的最佳临界值.方法 597例患者定为实验组,其中RA患者124例,非RA患者473例,以200例健康体检者为正常组,同时采用乳胶法和比浊法测定其血清RF,使用SPSS统计软件对检测结果进行分析并绘制比浊法的ROC曲线.结果 乳胶凝集试验的灵敏度、特异度分别为82.55%、87.76%,速率散射比浊法的灵敏度、特异度分别为95.16%、98.31%,其测定RF的最佳临界值是40 IU/mL,比传统以20 IU/mL作为正常参考值相比,特异度明显升高.结论 两种方法的检测结果呈较好的一致性,后者测定RF对RA的诊断价值更高,建议用40 IU/mL作为RA诊断的界限值. |

| Objective To detect rheumatoid factor by applying latex agglutination test and rate nephelometry, then compare the diagnostic value of the two methods for patients with rheumatoid arthritis and obtain the optimal cut - off value of turbidmetric assay in diagnosing RA. Methods Tests for rheumatoid factor were performed by the two methods in our rhetunatologic clinic among the outpatients and inpatients from July 2005 to June 2006 and 200 healthy cases were made as normal control. The receiver operating characteristics curves were drawn after analyzing the results by using statistical software. Results The sensitivity of latex agglutination test and rate nephelometry were 82.55% ,95.16% respectively, The specificity of them is 87.76% ,98.31% respectively, the optimal cut - off value of turbid metric assay in detecting RF was 40IU/ml and the specificity was higher in contrast with traditional nomaal referent value. Conclusions The results of the two methods are in good agreement, but the later has higher diagnostic value in RA, the optimal cut - off value of turbid metric assay in diagnosing RA is 40 IU/mL. |
