| 目的观察冠心病患者支架置入术后基质金属蛋白酶-9（MMP-9）、P-选择素血清浓度的动态变化，比较常规支架术和直接支架术后MMP-9、P-选择素血清浓度变化的差异。方法选择冠状动脉A、B型病变患者行常规支架术和直接支架术各23例，比较两组间在支架植入前，植入后即刻，术后4小时MMP-9、P-选择素血清浓度的差异。结果在支架植入前，常规支架组和直接支架组MMP-9、P-选择素血清浓度差异无显著性。支架植入后即刻血清浓度开始升高，4h升高明显，两组间差异有显著性（P〈0．01）。结论由于常规支架术反复的球囊预扩张，内膜损伤重，术后炎症反应剧烈，且血小板激活更明显。因此，对一些合适病变，应首先考虑直接支架术。 |

| Objective To compare the serum levels of matrix metalloproteinases-9(MMP-9) and P-selection between conventional stenting and direct stenting by measuring the dynamic changes of serum levels of matrix metalloproteinases-9(MMP-9) and P-selection after stenting.Methods Twenty three patients inserted with conventional stenting and twenty three patients inserted with direct stenting with A/B type coronary lesions after coronary angioplasty were investigated.Blood samples were collected before inserting stents,immediately,and 4 hours after inserting stents,respectively.The serum levers of MMP-9 and P-selection were detected with ELISA.Results There was not significant statistical difference of the serum levels of MMP-9 and P-selection in the 2 groups before inserting stents.However,the serum levels of MMP-9 and P-selection began to rise immediately after inserting conventional stenting.Their levels was significantly high 4 hours after inserting stents,and the difference in two groups were statistically significant(P<0.01). Conclusion The damage to intima by coronary angioplasty is more serious.The inflammation and platelet activation is more dramatic due to repeated dilation with balloon in conventional stenting group.Therefore,direct stenting should be first considered in some patients with appropriate coronary lesions. |
