| 目的 分析肝细胞癌（HGC）肝动脉-门静脉分流（APS）的多层螺旋CT动态增强表现，并探讨APS的形成原因及相关介入治疗方案的选择。方法 回顾性分析35例经肝动脉造影证实为HCC合并APS患者的肝脏螺旋CT动态增强表现，并与其肝动脉造影表现进行对比。结果 经肝动脉造影证实为合并APS的35例HCC患者中，CT增强动脉期表现分为三型：（1）门静脉主干和正常肝的门静脉分支在肝动脉期提前强化显影20例。（2）肿瘤同侧肝叶门静脉（2级或2级以远分支）在肝动脉期提前强化显影7例。（3）肿瘤周围非癌变肝实质在肝动脉期提前强化8例。结论 经血管、经肝窦、经肝血管丛的途径是肝细胞癌APS形成的主要机制。多层螺旋CT诊断HCC合并APS具有相当的优越性。 |

| Objective To analyze the dynamic enhanced multislice spiral CT findings of arterioportal shunt(APS) in hepatocellular carcinoma(HCC),and to study the cause of APS formation and the interventional therapy related to APS.Methods Dynamic enhanced multislice spiral CT findings in 35 HCC patients with angiographically proved APS were retrospectively analyzed,and the signs were compared with the hepatic angiographic results.Results 35 cases with APS could be divided into three enhancement types accoding to their CT signs in arterial phase(1)Early visualization of portal trunk and portal vein branches within normal hepatic area(n=20).(2) Early visualization of portal vein branches in the diseased lobe(n=7).(3) Early enhancement of the tumor or the hepatic parenchyma round the lesion(n=8).Conclusion The way of trans sinusoidal,trans vascular and trans vascular plexal growing route is the key mechanism of APS formation in HCC.Multi slice spiral CT has obvious advantages on the diagnosis of HCC with APS. |
