| 目的 探讨凋亡相关蛋白P53在不同人群、临床类型、病理分级的膀胱移行细胞癌（bladder transitional cell cancer，BTCC）中的表达，及其与影响膀胱肿瘤的预后因素（肿瘤数目、TNM分期、淋巴结转移、组织学分级等）的相关性。方法 应用免疫组织化学链霉抗生物素蛋白-过氧化物酶连接法（S—P法），检测P—gP、Bcl-2、p53在100例随访3年的膀胱移行细胞癌中的表达，同时以30例肾移植手术受者或前列腺增生患者的正常膀胱组织作对照。结果 p53在正常组织、初发性膀胱癌、复发性膀胱癌中的表达分别为6．7％、36．9％、71．4％。p53的表达在复发组高于初发组（P〈0．05）。p53的表达率随膀胱移行细胞癌的病理分级的升高而逐渐增高（P〈0．05），p53基因表达与膀胱移行细胞癌的临床分期呈正相关（P〈0．05）。总体复发率为46．2％。p53的表达水平均与无复发生存率及复发间隔时间密切相关（P〈0．05）。结论：（1）检测P53可能对膀胱移行细胞癌的预测复发有意义；（2）P53的高表达可以作为膀胱移行细胞癌分化不良的标志。 |

| Objective To evaluate the expression of p53 in the patients with bladder transitional cell cancer,and its relationship with the prognostic value in other prognostic factors which significantly influenced the disease free survival rate after surgical resection of primary BTCC(eg: clinical stage,lymph node,TNM grades,et al).Methods Using streptavidin-biotin peroxidase(S-P) method,the expression of diversity of p53,between 30 normal bladder tissues and 100 bladder transitional cell cancer tissues with over 3 years follow-up of various pathological type and clinical stage,were closely studied.Results The expression rate of p53 in the normal bladder tissue,primary BTCC,recurrent BTCC was 6.7%,36.9%,71.4% respectively(P<0.05).The expression of p53 in the recurrent BTCC was significantly higher than that in the primary BTCC(P<0.05).The rate of positive expression of p53 in the primary BTCC was positively correlated with the tumor grade and clinical stage.The overall recurrence rate is 46.2%.A strongly inverse relationship between the expression of p53 and survival times and T-free intervals could be demonstrated by Kaplan-Meier survival analysis(P<0.05).Conclusion (1)Detecting the expressin of p53 could contribute to the prediction of recurrence.(2)The over-expression of p53 could be used as the important markers of aggressiveness and prognosis of bladder transitional cell cancer. |
