| 目的 探讨食管心房调搏心脏负荷试验 (简称食管负荷试验 )时QT离散度 (QTd)的变化及其对冠心病的诊断价值。方法 对临床诊断或疑诊为冠心病的 42例患者先后行负荷试验及Judkins法冠脉造影 ;按造影结果分为冠心病组 ( 2 4例 )、冠脉正常组 ( 1 8例 ) ,测量并分析两组负荷试验前、后各时段QTd的变化。结果 冠心病组负荷试验前、后各时段QTd均大于冠脉正常组 (P <0 .0 5或 0 .0 1 ) ;冠心病组负荷试验后 0min、2min的QTd明显高于负荷试验前 (P均 <0 .0 1 ) ,而 4min、6min的QTd恢复至试验前水平 (P均 >0 .0 5 ) ;冠脉正常组负荷试验前、后各时段QTd差异无显著性 (P均 >0 .0 5 )。结论 QTd可作为食管心房调搏心脏负荷试验时诊断冠心病心肌缺血的有效指标之一。 |

| Objective To investigate the changes and diagnostic value of QT dispersion (QTd) for patients with coronary artery disease during cardiac stress testing (CST) by transesophageal atrial pacing (TEAP).Methods consecutive inpatients underwent CST by TEAP firstly, then accepted selective coronary angiography (SCA) by Judkins method. Patients were divided into two groups according the results of SCA, ie 24 cases in coronary artery disease group (CAD) and 18 cases with normal coronary artery (NCA). Measurements of QTd in two groups before and after CST were analyzed in and between groups. Results CAD group had a higher QTd than NCA group at baseline or every stages after CST (P<0.05 or 0.01); in CAD group, QTd immediately and 2 min after CST was significantly greater than before CST (P<0.01,respectively). In NCA group, there were no differences of QTd measurements to be observed before and every stages after CST (P>0.05, respectively). Conclusion QTd may be an effective index for myocardial ischemia during the test. |
