| 目的 判断贲门癌切除的可能性。方法 77例贲门癌患者术前行胃充气状态下用76％泛影葡胺100mL(离子型造影剂)做上腹部加强CT扫描。结果 CT预测55例能完成根治性手术切除的病例中，53例行根治性切除，2例行姑息性切除。预测22例不宜手术，结果 10例手术探查，10例姑息切除，2例根治性切除。其判断贲门癌是否根除手术的阳性预测准确率为90．9％，阴性预测准确率为96．4％。结论 术前胃充气状态下上腹部加强CT扫描对贲门癌的可切除性的估价有重要价值。 |

| objective To study the respectability of cancer of gastric cardia. Methods 77 preoperative patients received upper abdominal enhanced CT scan with gas- filled stomach. Results Among 55 patients whose tumor were respectable judged by CT scan, 53 received radical resection, 2 palliative operation. Among 22 patients whose tumor were unresectable judged by CT scan, 11 received exploration, 10 palliative operation and 2 radical resection. The positive expectation rate and negative expectation rate for the unresectability of cancer of gastric cardia is 90.9% and 96.4%. Conclusion preoperative upper abdominal CT scan with gas- filled stomach is of great value to judge the respectability of cancer of gastric cardia. |
