| 目的 提高对合并有腰骶椎管内良性肿瘤病变的腰椎间盘突出症患者的早期诊断率 ,避免漏诊和延误手术治疗。方法 回顾性分析 30例伴有腰骶椎管内肿瘤的腰椎间盘突出症病例的临床表现及手术和病理资料。结果 发病年龄 2 4～ 60a ,平均 4 2a。肿瘤以神经鞘瘤 (包括神经纤维瘤 )和脊膜囊肿多见。疼痛较剧且与体位有关 ,并有不同程度的马尾神经损伤表现是与单纯腰椎间盘突出症鉴别的重要体征。结论 CT扫描有局限性 ,易漏诊肿瘤病变 ,必须强调重视结合临床表现及MRI检查 ,才能早期作出正确的诊断以指导治疗 ,且手术疗效满意 |

| Objective:To improve the diagnosis and the treatment of the lumbar disc protrusion associated with benign spine cord tumor in lumbar-sacrum. Methods:30 cases of lumbar disc protrusion associated with benign spine cord tumor were reviewed in this article. The typical clinical characters, surgery methods and pathological characters were systematically analysed. Result:The age of the patients was fluctuated from 24 to 60 years old.The average age was 42. Most of the benign spine cord tumors were neurolemmoma(neurofibroma) and spine cord cyst. Most of the patients suffered from unbearable low back pain, especially when their body position are changed. In some cases, the injures of caudal equina are reported, which are typical syndrome different from the simple lumbar disc protrusion. Conclusion:The CT(computed temography)is limited in diagnosing this kind of disease. Emphasis should be put on typical characters and MRI(magnetic resonance imaging)characters in diagnosing and treating this kind of diseases. Surgical treatment is the most effective way to treat this kind of disease. |
