| M—超声心动图主动脉根部的活动能反映左房充盈和排空情况。本文对比研究了52例左室肥厚和29例正常对照的超声心动图,这52例左室肥厚者其病因有单纯性主动脉瓣狭窄、特发性肥厚性主动脉瓣下狭窄和原发性高血压。本研究目的是为了了解左室肥厚时早期快速充盈期改变是否影响主动脉根部后壁的活动。研究表明左室肥厚主动脉后壁M—超声心动图典型改变为:舒张早期正常的急剧后移减弱,左房排空指数(Atrial EmptyingIndex)明显降低(P<0.001)。所谓左房排空指数(AEI)是指主动脉后壁舒张期前1/3时间和整个被动排空期向后移位之比,利用AE1能识别左室肥厚充盈改变,其敏感性为92％,特异性为79％。AEI的降低很容易在超声心动图上观察到。因此是快速定性估测左室充盈特性的无创检查方法。 |

| The echocardiographic motion of the aortic root can reflect left atrial filling and emptying. The echocardiograms of 52 Patients with left ventricular hyPertroPhy (LVH) secondary to aortic stenosis, hypertrophic subaortic stenosis and hypertension were comopared with those of twentynine normal subjects to determine whether the alteration of early rapid diastolic filling associated with LVH would alter motion of the posterion aortic root. The study showed Patients with LVH had a characteristic echographic pattern of motion of the Posterior aortic root manifested by the attenuation of normal abrupt diastolic Posterior aortic root motion. The atrial emptying index (AEI) defined as the fraction of Passive Posterior aortic wall motion occuring in the first third of diastole was significantly reduced in Patients with LVH (P<0.001) The AEI may be useful in the identification of altered left ventricular filling associated with LVH (sensitivity: 92%; specificity: 79%). The changes associated with a decreased AEI are readily appreciated by visual inspection and provide a rapid method of qualitatively assessing left ventricular filling properties. |
