| 本文将本院1972年8月～1986年1月收治的24例急性异烟肼(INH)中毒的病例,根据其维生素 B_6(VB_6)的用量与疗效效应分成三组,作了比较性回顾;同时,对其发病机理及治疗进行了讨论.本文指出:尽早地一次静脉快速投入与吞服 INH 等剂量(克)的 VB_6,同时合并安定注射是急性 INH 中毒值得推荐的治疔方法,而烟酰胺(NAA)对抢救急性 INH 中毒并非重要。 |

| This paper presents a retrograde study of 24 cases of acute isoniazid overdose which can be catagorized into 3 groups according to the antidotal treatment with pyridoxine.The first group were treated with intravenous administration of pyridoxine hydrochloride in doses equivalent to the estimated amount of isoniazid ingested.The second group were given inadequate low dosage of B6.while in the third group no pyridoxine was given.The best therapeutic results were achieved in the first group.Nicotinamide was given simultaneously in some patients but proved to be of no importance in the treatment of acute poisoning of isoniazid. It seems to the author that the treatment of choice is to give a single intravenous dose of pyridoxine hydroehloride equal to the jestimated amount of isoniazid ingested with diazepam 5-10 mg injected intraveneously in the same time to potentiate the antidotal effects of 136 including seizure control.Victims of massive ingestion of isoniazid (10-15 gm in adults), usually fatal if untreated,were succesfully managed with the above mentioned measures.Convulsions ceased within 1.5-2.5 hours and cons- ciousness gradually recovered.The cured-improved rate observed in this study was 95%.The mechanisms of the isoniazid poisoning was also discussed. |
